研究目的：

总体目标 建立深圳市南山区社区老年综合征前瞻性队列，系统评估老年人健康与功能衰退相关结局的发生、演变及影响因素。 具体目标： 描述辖区 65 岁及以上常住居民认知功能下降、衰弱、肌少症和伤害事件的基线流行状况及随访变化趋势。 分析人口学特征、生活方式、既往病史、药物使用、体格和实验室指标与主要结局之间的关联。 探索不同老年综合征结局之间的时序关系、聚集模式与共同危险因素。 构建并验证主要结局的风险预测模型，为社区筛查分层和精准干预提供依据。 建立规范化数据库和生物样本资源平台，为后续机制研究和转化研究提供基础。  

Objectives of Study：

Overall Objective Establish a prospective cohort of community elderly syndromes in Nanshan District, Shenzhen, to systematically assess the occurrence, evolution, and influencing factors of health and functional decline outcomes in the elderly. Specific Objectives: Describe the baseline prevalence and follow-up trends of cognitive decline, frailty, sarcopenia, and injury events among residents aged 65 and above in the district. Analyze the associations between demographic characteristics, lifestyle, medical history, medication use, physical and laboratory indicators, and major outcomes. Explore the temporal relationships, clustering patterns, and common risk factors among different elderly syndrome outcomes. Construct and validate risk prediction models for major outcomes to provide a basis for community screening stratification and precise interventions. Establish standardized databases and a biosample resource platform to provide a foundation for subsequent mechanistic and translational research.

纳入标准：

①辖区居住满6个月及以上常住居民； ②具备完成问卷和体格检查的基本沟通与配合能力，或在近亲属协助下完成； ③自愿参加本研究并签署书面知情同意书。

Inclusion criteria

1. Permanent residents living in the community for more than 6 months; 2. The ability of communication and cooperation to complete the questionnaire and physical examination, or with the assistance of close relatives; 3. Voluntarily participated in this study and signed the written informed consent.

排除标准：

1. 因严重疾患、体弱或肢体残疾不能正常活动者； 2. 严重视听损伤、精神、认知或意识障碍不能正确回答问题者； 3. 拒绝签署知情同意书或明确表示不愿接受随访者。

Exclusion criteria：

1. Unable to perform normal activities due to severe illness, weakness or physical disability; 2. Severe visual and hearing impairment, mental, cognitive or conscious disorders who cannot answer questions correctly; 3. Refusing to sign informed consent or explicitly unwilling to be followed up.
