研究目的：

本研究旨在明确miR-129-5p/HMGB1炎症调控轴在多囊卵巢综合征（PCOS）中的表达特征及临床意义；对比优思悦单用、优思悦联合二甲双胍、三药联合GLP-1RA三种方案对PCOS患者体脂、糖代谢、性激素等指标的改善效果；验证二甲双胍联合GLP-1RA是否通过靶向调控该炎症轴发挥治疗作用，明确其协同治疗价值；同时评估miR-129-5p与HMGB1对PCOS的诊断预测价值，为优化PCOS临床联合用药策略、开发精准靶向治疗方案提供实验依据与理论支撑。  

Objectives of Study：

This study aims to clarify the expression characteristics and clinical significance of the miR-129-5p/HMGB1 inflammatory regulatory axis in polycystic ovary syndrome (PCOS). It will compare the improvement effects of three treatment regimens on body fat, glucose metabolism, sex hormones and other indicators in PCOS patients: Yousiyue (Drospirenone and Ethinylestradiol Tablets) alone, Yousiyue combined with metformin, and the triple combination of Yousiyue, metformin and GLP-1RA. Furthermore, it will verify whether metformin combined with GLP-1RA exerts therapeutic effects by targeted regulation of this inflammatory axis, and clarify its synergistic therapeutic value. Meanwhile, it will evaluate the diagnostic and predictive value of miR-129-5p and HMGB1 for PCOS, so as to provide experimental evidence and theoretical support for optimizing clinical combination medication strategies and developing precise targeted therapy regimens for PCOS.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

多囊卵巢综合征诊断标准：（1）月经异常如稀发排卵或无排卵，或不规则子宫出血（2）高雄激素相关临床表型和/或生化检测异常（3）PCOM（多囊卵巢形态）（1）是诊断的必须条件，再符合（2）和（3）中的一项2.年龄标准：18-45周岁。 健康体检者纳入标准：1. 既往身体健康，无慢性疾病相关病史；2. 于我院体检中心完成生化指标检测、影像学检查等项目，各项检查结果均在正常参考范围内；3. 年龄、性别等一般资料与病例组相匹配。

Inclusion criteria

1.Diagnostic criteria for polycystic ovary syndrome: (1) Abnormal menstruation, such as oligo-ovulation or anovulation, or irregular uterine bleeding; (2) Clinical manifestations related to hyperandrogenism and/or abnormal biochemical tests; (3) Polycystic ovarian morphology (PCOM). (1) is a necessary condition for diagnosis, and one of (2) and (3) shall also be met. Age criteria: 18–45 years of age. 2.Inclusion criteria for health checkup participants:(1).Generally healthy with no previous history of chronic diseases:(2). Completed biochemical tests, imaging examinations and other items at the Physical Examination Center of our hospital, with all results within normal reference ranges;(3).General data such as age and gender are matched with those of the case group.

排除标准：

门诊/住院病人排除标准：1.对GLP - 1RAs或MET过敏者；2.严重心血管疾病；3.肝功能检测结果异常( 谷丙转氨酶水平高于正常范围上限的2.5倍)；4.肾功能不全病史5.甲状腺功能异常；6.肿瘤病史；7.妊娠及哺乳； 健康体检者排除标准：1.确诊多囊卵巢综合征及其他内分泌疾病者；2. 近3个月使用过激素类药物、避孕药者；3. 月经严重紊乱、闭经或妊娠、哺乳期女性。

Exclusion criteria：

Outpatient/Inpatient Exclusion Criteria: 1.Patients with hypersensitivity to GLP-1RAs or MET; 2.Severe cardiovascular diseases; 3.Abnormal liver function test results (alanine transaminase level > 2.5 times the upper limit of normal range); 4.History of renal insufficiency; 5.Thyroid dysfunction; 6.History of malignant tumor; 7.Pregnancy and lactation. Exclusion Criteria for Health Checkup Participants: 1.Patients diagnosed with polycystic ovary syndrome and other endocrine diseases; 2.Individuals who have used hormonal drugs or contraceptives in the past 3 months; 3.Females with severe menstrual disorders, amenorrhea, pregnancy, or lactation.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

于包头医学院第一附属医院内分泌科门诊收集外周静脉血残血，qRT-PCR检测外周血miRNA-129-5p、HMGB1的表达，人工录入至Excel表格内建档记录数据；收集入组患者的年龄、身高、体重等一般资料及检测实验室检查指标，于包头医学院第一附属医院系统上获取结果，上传数据至Excel表格收集；待数据收集完毕后用SPSS 29.0进行数据分析，并用GraphPad Prism 10.0完成图表绘制。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Peripheral venous residual blood samples were collected from outpatients in the Department of Endocrinology, the First Affiliated Hospital of Baotou Medical College. The expression levels of miRNA-129-5p and HMGB1 in peripheral blood were detected by quantitative real-time polymerase chain reaction (qRT?PCR). Data were manually entered and documented in Excel spreadsheets.General data including age, height, weight, as well as laboratory examination indicators of the enrolled patients were collected. Results were obtained from the hospital information system of the First Affiliated Hospital of Baotou Medical College and uploaded to Excel for data collection.After data collection was completed, statistical analysis was performed using SPSS 29.0, and charting and graphing were conducted using GraphPad Prism 10.0
