研究目的：

阐明Cbl-b是否通过对海马小胶质细胞DBC1泛素化修饰，导致SIRT1蛋白增加，从而抑制NLRP3炎性小体活化及延缓老年PND的进展，为老年PND的发病机制及治疗策略提供理论依据。  

Objectives of Study：

To elucidate whether Cbl-b inhibits NLRP3 inflammasome activation and delays the progression of elderly PND by ubiquitinating hippocampal microglial DBC1, leading to an increase in SIRT1 protein, thereby providing a theoretical basis for the pathogenesis and treatment strategies of elderly PND.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

择期全身麻醉下行全髋关节置换术的老年患者（年龄≧65周岁）

Inclusion criteria

Elderly patients (aged ≥65 years) undergoing elective total hip arthroplasty under general anesthesia.

排除标准：

①术前麻醉门诊评估不通过； ②长期使用其他阿片类药物或止痛剂； ③合并肾脏或肝脏疾病、感染性疾病、心理疾病； ④有酗酒或药物滥用史；

Exclusion criteria：

1. Failed preoperative anesthesia assessment. 2. Long-term use of other opioids or analgesics. 3. Comorbid kidney or liver diseases, infectious diseases, or psychological disorders. 4. History of alcohol abuse or substance abuse.
