研究目的：

通过比较术前、术后CTA/CTP的变化以及术后连续监测TCD变化，探讨控制减压技术对重型颅脑损伤后颅内血管的保护作用及脑灌注的调节作用  

Objectives of Study：

By comparing the changes of CTA/CTP before and after operation and continuously monitoring the changes of TCD after operation, the protective effect of control decompression technique on intracranial blood vessels after severe craniocerebral injury and the regulatory effect of cerebral perfusion were discussed

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

① 直系亲属签署知情同意书意愿参与实验；
② 年龄18-75岁，不论性别；
③ 创伤性重症颅脑损伤，GCS评分3-8分之间；
④ 符合去骨瓣减压术的适应症。

Inclusion criteria

(1) The immediate family members signed the informed consent to participate in the experiment;
(2) 18-75 years old, regardless of gender;
(3) traumatic severe craniocerebral injury, GCS score between 3 and 8 points;
(4) in line with the indications of decompression of bone flap.

排除标准：

① 非创伤所致脑损伤，如溺水，心跳骤停，动脉瘤破裂，自发性脑出血等等；
② 手术禁忌症：长期服用抗凝血药物、基础疾病较多、入院生命体征不稳定，难以耐受手术、多器官功能衰竭等情况；
③ 特殊损伤部位，如脑干或脑室血肿；
④ 造影剂过敏、肾功能差等不能使用造影剂情况；
⑤ 入院GCS 3分，经过急诊治疗后无明显改善者；
⑥ 急诊脑干功能衰竭，如呼吸心跳难以维持；
⑦ 缺乏家庭成员对参与临床试验的同意或者患者参与其他临床试验的情况。

Exclusion criteria：

(1) Non-traumatic brain injury, such as drowning, cardiac arrest, aneurysm rupture, spontaneous cerebral hemorrhage, etc.;
(2) surgical contraindications: long-term use of anticoagulant drugs, more basic diseases, unstable vital signs upon admission, difficulty in tolerating surgery, multi-organ failure, etc.;
Special injury sites, such as brainstem or ventricular hematoma;
(4) contrast agent allergy, renal function is poor can not use contrast agent;
(5) admission GCS 3 points, after emergency treatment without obvious improvement;
(6) Emergency brain stem failure, such as respiratory heartbeat difficult to maintain;
(7) Lack of consent from family members to participate in clinical trials or participation of patients in other clinical trials.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Researchers should fill in the data to case report form online, and keep the data in line with the original records. The investigators will qury missing or incorrect data in the database and send them to the researcher in the form of query table. The missing or incorrect data will be modified after interpretation by researchers. Management of the database is carried out by the data manager according to the relevant operating procedures.
