研究目的：

本研究拟开展前瞻性、随机、对照、单盲临床研究，比较传统“填骨+覆膜”方法与“单纯填骨、不覆膜”改良方案在二壁骨缺损拔牙位点中的疗效差异，主要目标是验证无膜方案在骨量保持与软组织愈合等方面是否不劣于常规覆膜方案。  

Objectives of Study：

This study aims to conduct a prospective, randomized, controlled, single-blind clinical trial to compare the efficacy of the conventional “bone grafting with membrane coverage” approach and the modified “bone grafting without membrane coverage” protocol in extraction sites with two-wall bone defects. The primary objective is to determine whether the membrane-free approach is non-inferior to the conventional membrane-covered technique in terms of bone volume preservation and soft tissue healing.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥18岁，全身情况良好者；
2.患有严重牙周炎的单颗后牙，并且拔牙后缺牙区计划种植修复者；
3.拔牙窝的颊侧壁和腭/舌侧壁均存在骨丧失者；
4.拟拔牙区域存在邻牙；
5.口腔卫生良好：全口菌斑指数≤25%；全口出血指数小于等于25%；

Inclusion criteria

1.Age >=18 years and in good general health; 2.Single posterior tooth affected by severe periodontitis, with a plan for implant restoration in the extraction site. 3.Presence of bone loss on both the buccal and palatal/lingual walls of the extraction socket. 4.Adjacent teeth present in the planned extraction area. 5.Good oral hygiene: full-mouth plaque index <=25% and full-mouth bleeding index <=25%.

排除标准：

1.重度吸烟者（＞10只/日）；
2.长期用药需要（如免疫抑制剂、苯妥英钠等影响愈合的药物）；
3.怀孕或哺乳期妇女；
4.过去5年内有恶性肿瘤放疗或化疗史；
5.邻牙金属冠修复或 CBCT 图像质量不佳等导致CBCT无法完成配准者。

Exclusion criteria：

1.Heavy smokers (>10 cigarettes/day);
2.Long-term use of medications affecting healing (e.g., immunosuppressants, phenytoin, etc.);
3.Pregnant or breastfeeding women;
4.History of radiotherapy or chemotherapy for malignant tumors within the past 5 years;
5.Patients with adjacent teeth restored with metal crowns or those whose CBCT image quality is insufficient for registration;
