研究目的：

本研究旨在通过两个阶段的工作，探索老年住院患者中低风险导管非计划拔管（UEX）的防控方法，具体目的如下： 1.构建风险预测模型：利用回顾性临床数据，分析老年住院患者中低风险导管UEX的相关因素，构建一个风险预测列线图模型，并为模型的临床应用提供初步的内部验证。 2.初步探索干预方案：基于模型分析结果所提示的关键风险因素，通过文献检索和临床护士的经验访谈，初步形成一套针对性的UEX预防护理方案草案，以期为未来的应用研究提供参考。  

Objectives of Study：

This study aims to explore prevention and control methods for low-risk unplanned extubation (UEX) in elderly hospitalized patients through two stages of work, with specific objectives as follows: 1. Construct a risk prediction model: Using retrospective clinical data, analyze the factors related to low-risk catheter UEX in elderly hospitalized patients, build a risk prediction nomogram model, and provide preliminary internal validation for the clinical application of the model. 2. Preliminary exploration of intervention programs: Based on the key risk factors indicated by the model analysis results, conduct a literature review and interviews with clinical nurses to initially form a draft of a targeted nursing plan for the prevention of UEX, aiming to provide reference for future application research.

None (This is a non-interventional observational study, no drugs or treatment protocols are involved)  

纳入标准：

年龄≥60岁；住院期间留置胃管、尿管、深静脉置管等中低风险导管；病历资料完整。

Inclusion criteria

Age >= 60 years; placement of gastric tube, urinary catheter, or central venous catheter and other medium- to low-risk catheters during hospitalization; complete medical record information.

排除标准：

住院时间＜24小时；转科患者；因病情需要提前拔管的患者。

Exclusion criteria：

Hospitalization time < 24 hours; patients transferred to another department; patients who need early extubation due to their condition.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1. 数据采集：采用回顾性提取电子病历数据 + 质性访谈记录方式，由 2 名经过培训的护理研究生独立完成数据提取与录入； 2. 数据管理：使用 Excel 建立病例记录表（CRF），进行双人双录入与交叉核对，10% 样本行一致性检验（Kappa≥0.75）； 3. 数据清洗：缺失值 < 10% 采用多重插补法处理，≥10% 予以剔除； 4. 数据安全：所有数据去标识化处理，存储于医院加密服务器，仅研究团队成员可访问，严格遵守伦理保密要求。

1. Data collection: Retrospective extraction of electronic medical record data + qualitative interview records, independently completed by 2 trained graduate nursing students; 2. Data management: Excel-based Case Record Form (CRF) with double entry and cross-checking by two researchers, consistency test (Kappa ≥ 0.75) for 10% of samples; 3. Data cleaning: Multiple imputation for missing values < 10%, exclusion for missing values ≥ 10%; 4. Data security: All data are de-identified, stored on the hospital's encrypted server, accessible only to the research team, and strictly comply with ethical confidentiality requirements.
