研究目的：

本研究拟在传统中医理论“培土生金”指导下，探究针药结合对VIDD患者膈肌功能的改善作用及相关临床疗效。  

Objectives of Study：

Guided by the traditional Chinese medicine theory of "Reinforcing Earth to Generate Metal" (Pei Tu Sheng Jin), this study aims to investigate the ameliorative effects of combined acupuncture and herbal medicine on diaphragmatic function in patients with ventilator-induced diaphragmatic dysfunction (VIDD) and to evaluate the associated clinical efficacy.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 符合临床西医诊断及中医证候的机械通气患者； 2. 18岁≤年龄≤85岁，性别不限； 3. 可配合针刺及中药治疗，且对治疗采用的药物无过敏史； 4. 患者或监护人自愿参加和配合本研究，并签署知情同意书。

Inclusion criteria

1. Patients receiving mechanical ventilation who meet the clinical diagnostic criteria of Western medicine and the pattern identification criteria of Traditional Chinese Medicine (TCM); 2. Aged between 18 and 85 years, regardless of gender; 3. Able to cooperate with acupuncture and herbal medicine treatment, with no history of allergy to the medications used in the study; 4. The patient or their legal guardian voluntarily agrees to participate in and comply with this study, and has signed the informed consent form.

排除标准：

1. 穴位局部皮肤破损、感染； 2. 血流动力学不稳定，心率＞140次/min，收缩压＜90 mmHg； 3. 使用血管活性药物：血管加压素、任何剂量的肾上腺素、多巴胺或多巴酚丁胺剂量＞5 μg/(kg·min)、盐酸去甲肾上腺素＞0.2 μg/(kg·min)； 4. 心电图提示危及生命的心律失常； 5. 需要进一步手术或持续镇静； 6. 预期存活时间＜6个月或采用姑息治疗的患者； 7. 参加其他与机械通气相关研究者。

Exclusion criteria：

1. Local skin damage or infection at the acupuncture site; 2. Hemodynamic instability, with heart rate >140 beats/min or systolic blood pressure <90 mmHg; 3. Use of vasoactive drugs: vasopressin, any dose of epinephrine, dopamine or dobutamine at a dose >5 μg/kg/min, or norepinephrine >0.2 μg/kg/min; 4. Electrocardiogram indicating life-threatening arrhythmias; 5. Requirement for further surgery or continuous sedation; 6. Expected survival time of less than 6 months or patients receiving palliative care; 7. Participation in other studies related to mechanical ventilation.

由统计人员用统计软件生成随机数字表，由与本研究无关的专人按随机数字表将对应的分组代码装入信封并密封（拆封后不可复原），临床医生依次纳入受试者，并按照纳入时的顺序号拆开信封，根据信封内分组代码将受试者随机分入4组，定义1=对照组，2=中药组，3=针刺组，4=针药组。

A random number table was generated by a statistician using statistical software. An individual not involved in this study placed the corresponding grouping codes into envelopes according to the random number table and sealed them (the envelopes could not be resealed after opening). Clinicians enrolled the subjects sequentially and opened the envelopes according to the enrollment sequence numbers. The subjects were randomly divided into 4 groups based on the grouping codes inside the envelopes, defined as follows: 1 = control group, 2 = herbal medicine group, 3 = acupuncture group, and 4 = combined acupuncture and herbal medicine group.
