研究目的：

本研究旨在基于床旁超声技术，通过动态监测机械通气患者胃动力变化，识别重症患者肠内营养喂养方式转变过程中的喂养风险，为患者制定个性化营养方案提供依据。  

Objectives of Study：

This study aims to utilize bedside ultrasound technology to dynamically monitor gastric motility parameters in mechanically ventilated patients. By identifying feeding risks during the enteral nutrition transition period in critically ill patients, it seeks to provide a basis for developing individualized nutritional regimens.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥18岁； 2.预期住ICU时间>=3天； 3.机械通气≥48小时且预计需持续≥72小时； 4.有经胃管肠内营养指征的患者； 5.获知情同意。

Inclusion criteria

1.Age >= 18 years; 2.Expected ICU stay >= 3 days; 3.Mechanical ventilation >= 48 hours and expected to require continued ventilation for >= 72 hours; 4.Patients with an indication for gastric tube enteral nutrition (i.e., meeting the criteria for enteral feeding via gastric tube); 5.Informed consent obtained.

排除标准：

1.胃肠结构不完整的患者； 2.严重胃肠胀气超声下胃窦不能显影； 3.孕期及围产期妇女。

Exclusion criteria：

1.Patients with incomplete gastrointestinal anatomy (i.e., structural discontinuity or abnormality preventing normal function); 2. Severe gastrointestinal distension with non-visualization of the antrum on ultrasound; 3. Pregnant or perinatal women.

遵医嘱给予肠内营养，在管饲肠内营养时根据患者临床表现和症状，每4小时评估一次肠内营养耐受评分 ，根据肠内营养耐受评分调节喂养速度，总分为0分，每4h上调营养泵速度，直至不超过120ml/h,1～2分，维持原速度，3～4分，继续肠内营养，并减慢泵入速度至原来的50%，2h后重新评估；>=5分，暂停肠内营养，给予鼻肠管置入。直到达到目标喂养量后持续喂养至出院或转科。

干预措施代码：

Intervention：

Enteral nutrition is administered as prescribed. During tube feeding, the Enteral Nutrition Tolerance Score (ENTS) is assessed every 4 hours based on the patient's clinical presentation and symptoms. The feeding rate is adjusted according to the ENTS score as follows: Score 0: Increase the feeding pump rate every 4 hours (do not exceed 120 ml/h). Score 1-2: Maintain the current feeding rate. Score 3-4: Continue enteral nutrition but reduce the infusion rate to 50% of the previous rate. Reassess after 2 hours. Score ≥5: Suspend enteral nutrition and place a postpyloric tube. Continue enteral nutrition until the target feeding volume is achieved. Maintain feeding at the target volume until the patient is discharged or transferred to another department.
