研究目的:

目前牙片屏障术联合即刻种植术的疗效临床证据不足,本研究为进一步提供牙片屏障即刻种植术的临床循证证据,规范临床操作,建立临床共识。  

Objectives of Study:

At present, clinical evidence regarding the efficacy of socket shield technique combined with immediate implant placement is insufficient. This study aims to provide further evidence-based clinical data on immediate implant placement using this technique, to standardize clinical procedures, and to establish clinical consensus.

纳入标准:

1.因外伤或牙体牙髓病变导致上前牙无法保留而要求种植修复的患者 2.18 岁以上患者 3.唇侧骨板完整 4.牙根位于牙槽骨内情况适宜即刻种植者 5.拟拔除患牙没有大于 3 mm 的深牙周袋,没有直径超过 1 cm 的根尖病变 6.拥有自主能力,能理解和遵循手术流程,同意书面知情同意

Inclusion criteria

1.Patients requiring implant-supported restoration because maxillary anterior tooth/teeth cannot be preserved due to trauma or endodontic/pulpal disease. 2.Patients aged 18 years or older. 3.Intact buccal/ labial bone plate. 4.The root is positioned within the alveolar bone and the site is suitable for immediate implant placement. 5.The tooth planned for extraction has no deep periodontal pocket > 3 mm and no periapical lesion larger than 1 cm in diameter. 6.The patient has full decision-making capacity, can understand and comply with the surgical procedure, and provides written informed consent.

排除标准:

1.深覆合等咬合不利于即刻种植情况者 2.周状况不佳或治疗后无法控制者 3.系统性全身疾病或局部疾病会影响术后愈合或骨结合 4.治疗依从性差

Exclusion criteria:

1.Patients with unfavorable occlusal conditions for immediate implant placement, such as deep overbite. 2.Patients with poor periodontal conditions or whose periodontal status cannot be controlled after treatment. 3.Patients with systemic or local diseases that may impair postoperative healing or osseointegration. 4.Poor treatment compliance.

截除冠部牙体组织,在牙根上种植体植入的理想位置定点,再逐级备洞至合适的直径和长度。去除腭侧部分牙齿,保留唇侧牙片,去除所有残留的牙髓组织及填充材料,修整牙片厚度至约 1 mm。选择合适直径和长度的种植体(Element, Thommen Medical AG, Grenchen, 瑞士)植入,保证种植体的初期稳定性。种植体植入后,在唇侧间隙内植入胶原骨材料。

Intervention:

After tooth decoronation, the remaining root was reduced to approximately 1 mm above the alveolar crest using a diamond bur. The planned implant position was marked within the root and alveolar bone using a round bur, followed by sequential osteotomy preparation to the intended implant diameter and length. After osteotomy, the root fragment and socket walls were carefully inspected. All residual pulp tissue and any endodontic filling material were removed. The mesial, distal, and palatal portions of the root were then removed, while the mid-portion of the labial root segment was retained and shaped to a thickness of approximately 1 mm. An implant of appropriate diameter and length was placed, and the labial gap between the retained labial root fragment and the implant was grafted with a collagenated bovine bone substitute (Bio-Oss Collagen, Geistlich Biomaterials).

进行微创不翻瓣拔牙,彻底清理拔牙窝,并用无菌盐水冲洗。通过窝内探诊仔细检查唇侧骨壁的完整性。偏腭侧进行备洞并植入种植体。选择合适直径和长度的种植体植入,胶原骨填充种植体和颊侧骨壁之间的间隙,及种植体肩部以上的缝隙中填。

Intervention:

A minimally invasive, flapless tooth extraction was performed. The extraction socket was thoroughly debrided and irrigated with sterile saline. The integrity of the buccal bony wall was carefully assessed by intra-socket probing. Osteotomy preparation and implant placement were performed toward the palatal side. An implant of appropriate diameter and length was inserted, and a collagen-based bone graft material was packed into the gap between the implant and the buccal bony wall, as well as into the space above the implant shoulder.
