研究目的：

子宫内膜蜕膜化是胚胎着床和妊娠维持的关键环节。蜕膜化缺陷会导致胚胎着床失败、甚至胚胎发育障碍，从而引起反复种植失败、复发性自然流产、胎盘功能不全等妇产科疾病。目前临床诊断主要依赖病理学和激素水平检测，缺乏针对性分子标志物和干预靶点。本研究从代谢-表观遗传学角度切入，有望揭示复发性流产新的分子机制，为诊疗提供潜在靶点。  

Objectives of Study：

Endometrial decidualization is a crucial step in embryo implantation and pregnancy maintenance. Decidualization defects can lead to failed embryo implantation and even embryo development disorders, thereby causing repeated implantation failures, recurrent spontaneous abortions, placental insufficiency and other gynecological and obstetric diseases. At present, clinical diagnosis mainly relies on pathology and hormone level detection, lacking targeted molecular markers and intervention targets. This study, from the perspective of metabolism-epigenetics, is expected to reveal new molecular mechanisms of recurrent spontaneous abortion and provide potential targets for diagnosis and treatment.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.不同月经周期育龄女性组纳入标准： （1）女性，年龄20-40周岁（含20和40周岁）。 （2）排卵周期21-35天（含21和35天）。 （3）非复发性流产因素行宫腔镜检查或子宫手术患者。 （4）取样前3个月无激素治疗史、无输卵管及卵巢手术史。 2.RSA组纳入标准 （1）女性，年龄20-40周岁（含20和40周岁）。 （2）排卵周期21-35天（含21和35天）。 （3）早期妊娠（妊娠周数≤12周）并经超声检查确认诊断为稽留流产或难免流产，需要行人工流产手术的患者，且符合复发性自然流产的经典定义，即连续发生≥2次的妊娠失败。 （4）正常的卵巢储备和内分泌水平（FSH＜10 mIU/mL，LH＜10 mIU/mL，E2＜50 pg/mL）。 3.有既往妊娠史的人工流产对照组纳入标准 （1）女性，年龄20-40周岁（含20和40周岁）。 （2）排卵周期21-35天（含21和35天）。 （3）正常宫内早期妊娠，因非医学原因（如个人计划、社会因素等）自愿要求终止妊娠并行人工流产手术的健康女性，且有至少1次足月产或正常妊娠史，且无任何自然流产史、死胎、死产等不良孕产史。 （4）正常的卵巢储备和内分泌水平（FSH＜10 mIU/mL，LH＜10 mIU/mL，E2＜50 pg/mL）。

Inclusion criteria

1. Inclusion criteria for the group of women of childbearing age with different menstrual cycles: (1) Female, aged 20 to 40 years old (inclusive). (2) The ovulation cycle is 21 to 35 days (inclusive of 21 and 35 days). (3) Patients with non-recurrent miscarriage factors who underwent hysteroscopy or uterine surgery. (4) There was no history of hormone therapy or fallopian tube or ovarian surgery in the three months prior to sampling. 2. Inclusion criteria for the RSA group: (1) Female, aged 20 to 40 years (inclusive). (2) The ovulation cycle is 21 to 35 days (inclusive of 21 and 35 days). (3) Patients with early pregnancy (gestational weeks <=12 weeks) who have been confirmed by ultrasound to have missed abortion or inevitable abortion and need to undergo artificial abortion surgery, and who meet the classic definition of recurrent spontaneous abortion, that is, having experienced >=2 consecutive pregnancy failures. (4) Normal ovarian reserve and endocrine levels (FSH < 10 mIU/mL, LH < 10 mIU/mL, E2 < 50 pg/mL). 3. Inclusion criteria for the control group of induced abortion with a previous pregnancy history (1) Female, aged 20 to 40 (inclusive). (2) The ovulation cycle is 21 to 35 days (inclusive of 21 and 35 days). (3) Healthy women with normal early intrauterine pregnancies who voluntarily request to terminate the pregnancy and undergo artificial abortion due to non-medical reasons (such as personal plans, social factors, etc.), and have at least one history of full-term delivery or normal pregnancy, without any history of spontaneous abortion, stillbirth, stillbirth or other adverse pregnancy and childbirth. (4) Normal ovarian reserve and endocrine levels (FSH < 10 mIU/mL, LH < 10 mIU/mL, E2 < 50 pg/mL).

排除标准：

1.解剖结构异常：经超声、宫腔镜或造影等检查证实存在子宫畸形（如纵隔子宫、双角子宫）、Asherman综合征（宫腔粘连）、子宫肌瘤（直径>4cm或粘膜下肌瘤）等可能影响胚胎着床的解剖学因素。
2.染色体异常：夫妇任一方有染色体核型分析明确的异常。
3.内分泌疾病：患有控制不佳或未经治疗的严重内分泌疾病，如： 甲状腺功能异常（甲亢或甲减，TSH显著超出正常范围） 未控制的糖尿病（空腹血糖≥7.0 mmol/L） 免疫系统疾病：患有明确的自身免疫性疾病，如抗磷脂综合征（APS）、系统性红斑狼疮（SLE）、未分化结缔组织病等，或相关自身抗体（如抗心磷脂抗体、抗β2-GP1抗体、狼疮抗凝物）持续阳性。
4.感染因素：本次流产由生殖道感染（如衣原体、支原体、TORCH等）引起。
5.血栓前状态：存在已知的、与流产相关的易栓症（如蛋白S/C缺乏、抗凝血酶III缺乏等）。
6.其他重大疾病：合并严重的心、肝、肾、血液系统等重大原发性疾病，或精神疾病无法配合者。
7.药物影响：近期（如3个月内）有服用可能影响代谢或免疫功能的药物史（如糖皮质激素、二甲双胍、胰岛素、减肥药等）。
8.生活方式极端：有长期酗酒、吸烟、吸毒等不良嗜好。
9.组织样本不合格：手术获取的内膜组织或蜕膜组织量过少或因处理不当无法用于后续研究。
10.术后病理提示恶性肿瘤。

Exclusion criteria：

1. Anatomical structural abnormalities: The presence of uterine malformations (such as septate uterus, bicornuate uterus), Asherman syndrome (intrauterine adhesions), uterine fibroids (with a diameter of 4cm or submucosal fibroids), and other anatomical factors that may affect embryo implantation is confirmed by examinations such as ultrasound, hysteroscopy or angiography. 2. Chromosomal abnormalities: Either spouse has a clear abnormality identified by karyotype analysis. 3. Endocrine diseases: Suffering from poorly controlled or untreated severe endocrine diseases, such as: Abnormal thyroid function (hyperthyroidism or hypothyroidism, with TSH significantly exceeding the normal range) Uncontrolled diabetes (fasting blood glucose >=7.0 mmol/L) Immune system diseases: Suffering from definite autoimmune diseases, such as antiphospholipid syndrome (APS), systemic lupus erythematosus (SLE), undifferentiated connective tissue disease, etc., or persistently positive related autoantibodies (such as anticardiolipin antibodies, anti-β 2-GP1 antibodies, lupus anticoagulants). 4. Infection factors: This miscarriage was caused by reproductive tract infections (such as chlamydia, mycoplasma, TORCH, etc.). 5. Pre-thrombotic state: There are known thrombophilia related to miscarriage (such as protein S/C deficiency, antithrombin III deficiency, etc.). 6. Other major diseases: Those with severe primary diseases of the heart, liver, kidneys, blood system, etc., or those who cannot cooperate with mental disorders. 7. Drug impact: There is a recent history (such as within 3 months) of taking drugs that may affect metabolism or immune function (such as glucocorticoids, metformin, insulin, weight loss drugs, etc.). 8. Extreme lifestyle: Has long-term bad habits such as heavy drinking, smoking and drug abuse. 9. Unqualified tissue samples: The amount of endometrial tissue or decidua tissue obtained during the surgery is too small or cannot be used for subsequent research due to improper handling. 10. Postoperative pathology suggests a malignant tumor.
