研究目的：

1.构建袋鼠式照护联合中医五行音乐疗法对NICU早产儿的干预方法，探讨该干预方案对早产儿神经行为发育、体格发育；对早产儿母婴依恋关系、早产儿母亲育儿胜任力的应用效果。 2.验证该干预方案对NICU早产儿的可行性。  

Objectives of Study：

1. Develop a combined intervention approach integrating kangaroo care with Traditional Chinese Medicine Five Elements music therapy for preterm infants in the NICU. This study aims to evaluate the intervention's effectiveness on neurobehavioral development, physical growth, mother-infant attachment, and maternal parenting competence among preterm infants. 2. Validate the feasibility of this intervention protocol for preterm infants in the NICU.

纳入标准：

符合早产儿诊断标准：胎龄小于37周，体重小于2.5 kg；出生3d内入院；早产儿生命体征平稳；早产儿听力正常（通过听力筛查者）；早产儿母亲身体健康，无言语沟通障碍；知情同意且自愿参与本研究。

Inclusion criteria

Meets the diagnostic criteria for preterm infants: gestational age less than 37 weeks, birth weight less than 2.5 kg; admitted within 3 days of birth; stable vital signs; normal hearing (passed hearing screening); mother in good health with no speech communication barriers; informed consent obtained and voluntary participation in this study.

排除标准：

早产儿合并复杂性先天性疾病、严重遗传代谢性疾病、严重脑损伤：、严重窒息；早产儿患有需特殊体位的疾病或治疗需要特殊体位；早产儿母亲产后需严格卧床，难以开展袋鼠式母亲照护。

Exclusion criteria：

Premature infants with complex congenital anomalies, severe inherited metabolic disorders, or severe brain injury; Premature infants with conditions requiring specific positioning or treatments necessitating specific positioning; Premature infants whose mothers require strict bed rest postpartum, making kangaroo care difficult to implement.

建立干预小组 ，袋鼠式护理时同时实施五行音乐疗法，每周3次，每次30—45分钟，为期4周。实施五行音乐疗法时将专用播放器统一放置在床旁吊柱上，与母亲及早产儿双耳保持15～25cm距离，播放音乐的分贝控制在50～55dB之间。

Establish an intervention group to implement Five Elements music therapy concurrently with kangaroo care three times weekly for 30–45 minutes per session over a four-week period. During music therapy sessions, dedicated players shall be uniformly mounted on bedside pole stands, positioned 15–25 cm from both the mother's and preterm infant's ears. Music volume shall be maintained between 50–55 dB.

实施为期4周的袋鼠护理，每周3次，每次30—45分钟。实施袋鼠式护理前护士核对早产儿母亲身份，告诉步骤并取得配合；早产儿母亲穿隔离衣、戴口罩，流水洗手；护士备躺椅、用屏风遮挡，协助早产儿母亲斜躺（30-45度），后背垫靠枕；将早产儿（仅穿纸尿裤）放于其母亲前胸（皮肤贴皮肤）、头偏向一侧，手臂屈放身侧或母亲前胸，膝关节屈曲成“青蛙”位；早产儿母亲托住早产儿臀部，护士用包被盖住早产儿后背；持续监测早产儿生命体征；告知早产儿母亲操作中自身或早产儿有不适需及时告知医护；指导母乳喂养的正确方法。

Intervention：

Implement kangaroo care for 4 weeks, 3 times per week, each session lasting 30–45 minutes. Before initiating kangaroo care, nurses verify the premature infant's mother's identity, explain the procedure, and obtain consent. The mother puts on isolation gowns and masks, then performs hand hygiene with running water. The nurse prepares a recliner and screens off the area, assisting the mother to recline at a 30–45-degree angle with a pillow supporting her back. The preterm infant (wearing only a diaper) is placed skin-to-skin on the mother's chest, with the head turned to one side, arms flexed at the elbows resting alongside the body or on the mother's chest, and knees flexed in a "frog" position. The mother supports the infant's hips while the nurse covers the infant's back with a swaddle blanket. Vital signs are continuously monitored. The mother is instructed to promptly notify medical staff if she or the infant experiences discomfort during the process. Proper breastfeeding techniques are demonstrated and explained.
