研究目的：

鉴于 AI-HEALS 在慢性病自我管理中的潜力，本研究以多理论模型（MTM)为理论框架，采用 AI-HEALS 作为干预工具，旨在通过个性化和智能化的干预措施提高患者的自我管理能力。具体研究目标为： （1）评估该干预措施对至少同时患有以下两种疾病（高血压、2型糖尿病或高脂血症）患者生理指标的影响； （2）评估该干预措施对改善患者心理状态的影响（如：焦虑抑郁、自我效能感等）； （3）评价该干预措施对患者自我管理行为（如身体活动、用药依从性和饮食习惯）的效果。  

Objectives of Study：

The specific objectives are: (1) to assess the impact of the intervention on the physiological outcomes (blood pressure, blood glucose, and blood lipid) in patients diagnosed with at least two of the following conditions: hypertension, type 2 diabetes, or hyperlipidemia; (2) to assess how the intervention influences the improvement of self-management behaviors(e.g., anxiety, depression, self-efficacy, etc.); (3) To evaluate the effects of the intervention on patients’ self-management behaviors, such as physical activity, medication adherence, and dietary practices.

纳入标准：

1.年龄≥18岁；
2.符合高血压、2型糖尿病、高脂血症中至少两种疾病的诊断标准；
3.熟练使用智能手机，且能熟练操作微信等应用软件；
4.生活自理，有思维能力，语言表达清晰；
5.患者知情同意，自愿参与研究；

Inclusion criteria

1.Age >= 18 years; 2.Diagnosed with at least two of the following conditions: hypertension, type 2 diabetes, or hyperlipidemia; 3.Proficient in using smartphones and capable of operating applications such as WeChat; 4.Able to perform activities of daily living independently, with intact cognitive function and clear verbal communication; 5.Provided informed consent and voluntarily agreed to participate in the study.

排除标准：

1.存在严重的心、肝、肾等重要脏器功能障碍；
2.在过去六个月内接受过放疗或化疗；
3.正在接受其他临床研究；
4.妊娠期或哺乳期妇女；

Exclusion criteria：

1.Presence of severe dysfunction in major organs such as the heart, liver, or kidneys;
2.Received radiotherapy or chemotherapy within the past six months;
3.Currently participating in other clinical studies.
4.Pregnant or lactating women;

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

To streamline data entry, this study utilized the Wenjuanxing (a professional online survey platform) mini-program for data collection. Prior to collection, the electronic questionnaire was configured according to specific requirements to prevent missing responses or incorrect selections, ensuring data completeness. During the collection process, participants completed the questionnaires independently. For hospitalized patients, data collectors provided on-site guidance throughout the process—including instructions on completion and submission—and addressed any inquiries to ensure successful data submission and validity.Stringent data protection measures were implemented, including encrypted storage, access control, and anonymization, to safeguard participant privacy. Personal identifiers were managed separately from research data through encryption. Identity information was stored using encryption technology, with access restricted solely to the principal investigator. Furthermore, all data storage and transmission employed encryption protocols, with access strictly limited to authorized research personnel. Regular data backups were performed to protect research data from external threats and ensure security.
