系统评估氨甲环酸调控炎性反应对老年髋/膝关节置换术后谵妄发生发展的影响效应，深入探讨药物与炎性介质动态变化、神经认知功能损伤之间的关系，阐明氨甲环酸通过调控NF-κB等关键信号通路抑制过度炎性反应的分子机制。通过构建基于炎性生物标志物的术后谵妄风险预测模型，制定个体化精准干预策略，最终形成围术期炎性反应动态监测-药物精准调控-神经功能保护三位一体的综合管理方案，为改善老年患者术后认知结局提供循证医学依据与临床实践路径。  

Objectives of Study：

The system assesses the impact of tranexamic acid on the regulation of inflammatory responses on the occurrence and development of postoperative delirium in elderly patients undergoing hip or knee replacement surgery, and deeply explores the relationship between the dynamic changes of drugs and inflammatory mediators and neurocognitive function damage. It clarifies the molecular mechanism by which tranexamic acid inhibits excessive inflammatory responses through regulating key signaling pathways such as NF-κB. By constructing a postoperative delirium risk prediction model based on inflammatory biomarkers, individualized and precise intervention strategies are formulated. Ultimately, a comprehensive management plan integrating dynamic monitoring of perioperative inflammatory responses, precise drug regulation, and neuroprotective functions is formed, providing evidence-based medical evidence and clinical practice pathways for improving postoperative cognitive outcomes in elderly patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥65岁；（2）接受髋或膝关节置换手术；（3）ASAⅠ～Ⅲ级；（4）麻醉方式均为全身麻醉。

Inclusion criteria

1. Age>= 65 years; 2. Undergoing hip or knee joint replacement surgery; 3. ASA grade I to III; 4. All anesthesia methods were general anesthesia.

排除标准：

（1）患有中枢神经系统感染、脑卒中、头部创伤、癫痫、多发性硬化或其他主要神经系统疾病；（2）存在抑郁症、等严重心理疾病或者长期服用精神类药物、类固醇类药物和激素药物；（3）术前存在谵妄、痴呆等认知功能障碍；（4）1年内接受过重大手术；（5）病例资料不完整，未按时完成随访者。

Exclusion criteria：

1. Having central nervous system infections, stroke, head trauma, epilepsy, multiple sclerosis or other major neurological disorders; 2. Having severe psychological conditions such as depression or long-term use of psychotropic drugs, steroid drugs and hormone drugs; 3. Having cognitive dysfunction such as delirium or dementia before the surgery; 4. Having undergone major surgery within the past 1 year; 5. Having incomplete case data or failing to complete follow-up on time.
