研究目的：

目前，关于阻滞髂腹下神经（IHN）和髂腹股沟神经（IIN）对于全髋关节置换术（THA）术后镇痛效果的相关研究还很少。然而，髂腹下神经（IHN）和髂腹股沟神经（IIN）阻滞已经用于腹部手术。 鉴于目前全髋关节置换术（THA）的手术量很大，推测联合髂腹下神经（IHN）和髂腹股沟神经（IIN）阻滞对THA患者术后的镇痛效果和生活质量的恢复方面也具有很大的潜力。因此，我们设计了这项前瞻性、随机对照临床试验，假设髋关节囊周神经（PENG）阻滞联合股外侧皮神经、髂腹下神经（IHN）和髂腹股沟神经（IIN）阻滞对全髋关节置换（THA）患者术后镇痛效果较传统的周围神经阻滞更好，从而改善患者预后，指导临床工作。  

Objectives of Study：

At present, there are few studies on the effects of blocking the iliohypogastric nerve (IHN) and ilioinguinal nerve (IIN) on postoperative analgesia after total hip arthroplasty (THA). However, the block of the iliohypogastric nerve (IHN) and ilioinguinal nerve (IIN) has already been used in abdominal surgeries. Given the large volume of THA surgeries currently performed, it is speculated that the combined block of the iliohypogastric nerve (IHN) and ilioinguinal nerve (IIN) has great potential for improving the postoperative analgesia and recovery of quality of life in THA patients. Therefore, we designed this prospective, randomized controlled clinical trial, hypothesizing that the pericapsular nerve block (PENG) combined with the block of the lateral femoral cutaneous nerve, iliohypogastric nerve (IHN), and ilioinguinal nerve (IIN) would provide better postoperative analgesia for THA patients compared to traditional peripheral nerve blocks, thereby improving patient prognosis and guiding clinical practice.

纳入标准：

1.择期于全身麻醉下接受全髋关节置换术（THA）后外侧入路的患者 2.年龄≥18岁 3.体重指数BMI≤35kg/m2 4.美国麻醉医师协会（ASA）I-III级

Inclusion criteria

1. Patients undergoing total hip arthroplasty (THA) via the posterolateral approach under general anesthesia 2. Age >= 18 years 3. Body Mass Index (BMI) <= 35 kg/m^2 4. American Society of Anesthesiologists (ASA) grade I-III

排除标准：

1.患者拒绝参与研究 2.认知功能障碍或沟通无效 3.肢体截肢、截瘫 4.存在影响下肢肌肉的相关疾病（如脊髓灰质炎、小儿麻痹症、重症肌无力、多发性肌炎等） 5.周围神经阻滞麻醉禁忌症（例如，穿刺部位皮肤感染、凝血功能异常或障碍等） 6.慢性疼痛、长期使用镇痛药物史或对阿片类药物成瘾 7.对局部麻醉药物过敏或不能耐受 8.使用其他手术入路 9.使用脊髓麻醉 10.双侧手术或关节置换术后翻修 11.目前参加了另一项可能影响主要结局的干预性研究

Exclusion criteria：

1. Patients refuse to participate in the study. 2. Cognitive dysfunction or ineffective communication. 3. Limb amputation or paraplegia. 4. Presence of related diseases affecting the lower limb muscles (such as poliomyelitis, myasthenia gravis, polymyositis, etc.). 5.Contraindications of peripheral nerve block anesthesia (such as skin infection at the puncture site,coagulopathy, etc.). 6. Chronic pain, history of long-term use of analgesics or addiction to opioid drugs. 7. Allergy or intolerance to local anaesthetic. 8. Undergoing other surgical approaches. 9. Patients under spinal anesthesia. 10. Bilateral surgery or revision after joint replacement. 11. Currently participating in another intervention study that may affect the primary outcome.

由独立研究者使用计算机生成随机数字表，将符合标准的患者按照1:1的比例进行随机分为2组:一组接受髋关节囊周神经（PENG）阻滞联合股外侧皮神经、髂腹下神经（IHN）和髂腹股沟神经（IIN）阻滞（试验组），一组仅接受髋关节囊周神经（PENG）阻滞联合股外侧皮神经阻滞（对照组）。

An independent researcher performs a computer to generate a random number table, and then randomly divided the patients meeting the criteria into two groups at a 1:1 ratio: one group received pericapsular nerve group (PENG) block combined with lateral femoral cutaneous nerve, iliohypogastric nerve (IHN), and ilioinguinal nerve (IIN) block (the experimental group), while the other group only received pericapsular nerve group (PENG) block combined with lateral femoral cutaneous nerve block (the control group).

盲法：

试验分组情况对患者、手术医生设盲。患者入手术室后，由一名麻醉医生打开密闭信封，并根据分组情况准备相关药物，并行神经阻滞。该麻醉医生不参与后续数据收集及数据分析。原则上，若不小心暴露分组，则更换负责数据采集及随访的研究者。

Blinding：

The grouping situation of the experiment is blinded to the patients and the surgeons. After the patients enter the operating room, an anesthesiologist opens the sealed envelope and prepares the relevant drugs according to the grouping situation, and performs nerve block. This anesthesiologist does not participate in the subsequent data collection and analysis. In principle, if the grouping is accidentally exposed, the researcher responsible for data collection and follow-up will be replaced.
