研究目的：

评估老年糖尿病患者在术前短时摄入清流质后(200ml)的胃内容物 体积、排空速度与误吸风险水平。  

Objectives of Study：

To evaluate the gastric content volume, gastric emptying rate, and aspiration risk level in elderly patients with diabetes mellitus after short-term preoperative ingestion of 200 ml of clear fluid.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 2 型糖尿病患者（符合空腹血糖>=7mmol/L 或随机血糖>=11.1mmol/L 且伴典型高血糖症状）或非糖尿病患者 2. 年龄>=65岁 3. 美国麻醉医师协会（ASA）身体状况分级为 I-III 级 4. 体重指数（BMI）<35kg/m2 5. 择期膝关节手术患者 6. 糖尿病组需符合：明确诊断 2 型糖尿病，病情稳定。 7. 非糖尿病对照组：无糖尿病史，空腹血糖与 HbA1c 均在正常范围。 8. 能够理解研究目的并签署知情同意书

Inclusion criteria

1. Patients with type 2 diabetes mellitus (fasting blood glucose >=7mmol/L or random blood glucose>=11.1mmol/L with typical symptoms of hyperglycemia) or non-diabetic patients 2. Age >=65 years old 3. American Society of Anesthesiologists physical status I-III 4. Body mass index (BMI) <35kg/m2 5. Patients undergoing elective knee surgery 6. The diabetic group should meet the criteria of definite diagnosis of type 2 diabetes mellitus and stable condition. 7. Non-diabetic control group: no history of diabetes, fasting blood glucose and HbA1c were in the normal range. 8. Be able to understand the purpose of the study and sign informed consent

排除标准：

1.既往胃、食管、十二指肠重大手术或结构异常；既往胃轻瘫确诊； 2.严重胃食管反流病； 3.近 1 个月内有上消化道出血史；GCSI（Gastroparesis Cardinal Symptom Index）≥18 分（提示潜在胃轻瘫者排除）； 4.近期（过去2周）使用促动力药或显著延缓胃排空的药物未按指南停药； 5.过去 2 周内存在胃肠炎、呕吐、腹泻、发热或任何急性疾病； 6.正在接受 GLP-1 受体激动剂治疗；近期使用大量阿片类镇痛药、抗胆碱药、三环类抗抑郁药或其他影响胃动力的药物（或记录并作为协变量）； 7.严重肾功能不全、活动性感染、认知不能配合、无法平卧或无法耐受胃超声检查者； 8.甲状腺功能严重异常（明显甲状腺功能减退或亢进）BMI ≥35 kg/m2： 9.怀孕或哺乳； 10.清液饮用前血糖 > 16.7 mmol/L 或 < 3.9 mmol/L 且无法在短时间内矫正者

Exclusion criteria：

1. Previous major surgery or structural abnormalities of the stomach, esophagus, or duodenum; Previous gastroparesis was diagnosed; 2. Severe gastroesophageal reflux disease; 3. The history of upper gastrointestinal bleeding in the past 1 month; GCSI (Gastroparesis Cardinal Symptom Index) >=18 (patients with potential gastroparesis were excluded); 4. The use of prokinetic drugs or drugs significantly delaying gastric emptying in the past 2 weeks was not stopped according to the guidelines; 5. Gastroenteritis, vomiting, diarrhea, fever, or any acute illness in the past 2 weeks; 6. Receiving GLP-1 receptor agonist therapy; Recent heavy use of opioid analgesics, anticholinergic drugs, tricyclic antidepressants, or other drugs affecting gastric motility (or recorded and included as a covariate); 7. Severe renal insufficiency, active infection, cognitive failure to cooperate, unable to lie flat or unable to tolerate gastric ultrasound examination; 8. Severely abnormal thyroid function (significant hypothyroidism or hyperthyroidism) BMI >=35 kg/m2 : 9. Pregnancy or breastfeeding; 10. Blood glucose > 16.7 mmol/L or < 3.9 mmol/L before drinking liquid and cannot be corrected in a short time

This was a prospective, non-randomized controlled study with 1:1 individual matching based on age (+/-5 years), sex, and body mass index (BMI) categories. Matching was performed to ensure comparability between patients with and without type 2 diabetes and to minimize potential confounding.
