多中心心血管血液动力学数值仿真工程分析( Multicenter Analysis of Cardiovascular Hemodynamics via IN-silico Engineering, MACHINE)是一项多中心联合进行的系列研究, 旨在通过多中心联合, 利用大数据优势探索流体数值仿真技术在循环系统疾病诊治中的应用价值,为临床实践提供可靠的理论依据和技术支持。 该研究为系列研究的第一个课题, 通过结合机器学习、计算流体力学仿真等技术对 EVAR 术后 2 型内漏的机制进行深度分析并尝试构建术前临床预测模型, 旨在高效、准确地预测 T2EL 的发生,并为术中预防性干预策略(如风险分支动脉弹簧圈栓塞、瘤腔纤维蛋白胶注射等) 提供理论指导依据, 进一步证实流体数值仿真技术在心血管疾病临床应用转化中的价值。该队列研究旨在通过探究髂内动脉栓塞与2型内漏之间的关联,为后续研究提供线索依据。 |

Objectives of Study: |
Multicenter Analysis of Cardiovascular Hemodynamics via in-silico engineering (MACHINE) is a multicenter collaborative research series aimed at leveraging the advantages of big data to explore the application value of computational fluid dynamics in the diagnosis and treatment of circulatory system diseases, providing a reliable theoretical basis and technical support for clinical practice. As the first study in this series, this project integrates machine learning and computational fluid dynamics simulation to conduct an in-depth analysis of the mechanisms of type II endoleaks (T2EL) after endovascular aortic repair (EVAR) and attempts to construct a preoperative clinical prediction model. The goal is to efficiently and accurately predict the occurrence of T2EL and provide a theoretical foundation for intraoperative preventive intervention strategies (such as risk branch artery coil embolization and aneurysm sac fibrin glue injection), further demonstrating the translational value of computational fluid dynamics in clinical applications for cardiovascular diseases. This cohort study aims to explore the association between internal iliac artery embolization and type II endoleaks, providing clues and evidence for subsequent research. |

1. Age 50-90 years 2. Undergoing EVAR for infrarenal true abdominal aortic aneurysm 3. Complete preoperative CTA imaging data available 4. At least two postoperative follow-up records with complete imaging data available (T1 <= 3 months, T2 >= 6 months) |

排除标准: |
1. 破裂腹主动脉瘤 2. 感染性腹主动脉瘤 3. 炎性腹主动脉 4. 涉及内脏分支重建或髂分支重建 5. 术后随访发现其他类型的内漏 6. 术后合并其他系统的严重并发症 7. 严重、罕见的血管畸形 8. 合并确诊的结缔组织病 9. 术前肠系膜下动脉血栓闭塞 10. 术中进行内漏预防性干预措施,如弹簧圈栓塞或纤维蛋白胶注射等 11. 术前或术后影像学质量较差,难以进行三维模型重建及后续 CFD 计算 |

Exclusion criteria: |
1. Ruptured abdominal aortic aneurysm 2. Infected abdominal aortic aneurysm 3. Inflammatory abdominal aortic aneurysm 4. Involvement of visceral branch reconstruction or iliac branch reconstruction 5. Detection of other types of endoleaks during postoperative follow-up 6. Presence of other severe comorbidities postoperatively 7. Severe, rare vascular malformations 8. Diagnosed connective tissue disease 9. Preoperative thrombotic occlusion of the inferior mesenteric artery 10. Intraoperative prophylactic interventions for endoleak, such as coil embolization or fibrin glue injection 11. Poor quality of preoperative or postoperative imaging, inadequate for 3D model reconstruction and subsequent CFD computations |
