研究目的:

1. 探索反复胚胎质差患者通过自体内膜上皮细胞共培养改善胚胎质量的可行性。 2. 通过整合代谢组学与蛋白质组学技术,系统鉴定内膜上皮细胞共培养体系中挽救胚胎质量的关键分子。探索这些关键分子的功能及其调控网络,为优化胚胎培养液配方、开发靶向干预策略提供直接的理论依据和候选分子。  

Objectives of Study:

1. Explore the feasibility of improving embryo quality in patients with recurrent poor embryo quality through co-culture with autologous endometrial epithelial cells. 2. By integrating metabolomics and proteomics technologies, systematically identify key molecules that rescue embryo quality in the endometrial epithelial cell co-culture system. Explore the functions and regulatory networks of these key molecules to provide direct theoretical foundations and candidate molecules for optimizing embryo culture medium formulations and developing targeted intervention strategies.

纳入标准:

1. 既往外院和我院>=2个助孕周期胚胎质量差患者,包括碎片胚胎率>=60%、胚胎卵裂期发育阻滞、卵裂期胚胎质量优良但囊胚形成率<=20%的患者; 2. 拟再次行辅助生殖助孕治疗; 3. 既往有移植失败史,同意行内膜搔刮术以改善内膜容受性; 4. AMH>=2.0; 5. 自愿签署知情同意书

Inclusion criteria

1. Patients with poor embryo quality in >=2 prior assisted reproduction cycles at other hospitals or our hospital, including those with fragmentation rate >=60%, arrest at cleavage stage, or good-quality cleavage-stage embryos but blastocyst formation rate <=20%; 2. Planning to undergo another round of assisted reproductive treatment; 3. History of failed embryo transfer, with consent to undergo endometrial scratching to improve endometrial receptivity; 4. AMH >=2.0; 5. Voluntarily signed informed consent form.

排除标准:

1. 使用供精或供卵助孕; 2. 拟行胚胎动态观测; 3. 正常受精卵(2PN)数<2; 4. 有妊娠禁忌症或辅助生育技术禁忌症

Exclusion criteria:

1. Use of donor sperm or donor eggs for assisted reproduction; 2. Planning to undergo time-lapse embryo monitoring; 3. Number of normally fertilized oocytes (2PN) <2; 4. Contraindications to pregnancy or contraindications to assisted reproductive technology.

本研究为一个 pilot 研究,拟前期招募 30 位患者 内膜上皮细胞共培养组: 按临床取卵计划提前1周复苏上皮细胞,50微升每滴加入60mm皿,使其贴壁生长,待上皮细胞生长汇合至80-100%(48-72h),用雌激素诱导24小时,再用孕激素+cAMP诱导24小时,使子宫内膜细胞处于容受性状态,共培养当天,用磷酸盐缓冲液(DPBS)清洗3-5遍子宫内膜细胞以去除培养基,更换G1.5培养基(Vitrolife),将正常受精卵与内膜细胞共培养。 常规培养组: 为对照组,不添加内膜细胞共培养,正常受精卵在G1.5培养基(Vitrolife)中常规培养

Intervention:

This is a pilot study aiming to enroll 30 patients initially. Endometrial epithelial cell co-culture group: Endometrial epithelial cells are thawed 1 week prior to oocyte retrieval, plated at 50 μL/drop in 60 mm dishes, and allowed to adhere and grow to 80–100% confluence (48–72 h). Cells are then induced with estrogen for 24 h, followed by progesterone + cAMP for another 24 h to establish a receptive state. On the day of co-culture, endometrial cells are washed 3–5 times with DPBS to remove residual medium, then replaced with G1.5 medium (Vitrolife). Fertilized zygotes are co-cultured with the endometrial cells. Conventional culture group: As the control, zygotes are cultured in G1.5 medium (Vitrolife) without endometrial cell co-culture.

使用动态区组随机化,区组长度为2/4,医院循证医学工作室人员使用R软件生成随机数表。授精后第一天原核评分后,将患者的正常受精卵(2PN)随机分入内膜上皮细胞共培养组和常规培养组。

Randomization Procedure (please state who generates the random number sequence and by what method):

By employing dynamic block randomization with block sizes of 2 or 4, personnel from the hospital's Evidence-Based Medicine Studio generated a random number table using R software. On the first day post-fertilization, following pronuclear scoring, the normally fertilized oocytes (2PN) from patients were randomly allocated to either the endometrial epithelial cell co-culture group or the conventional culture group.
