研究目的：

1、建立完善多中心老年糖尿病多代谢紊乱和肢体血管病变临床队列；进行中药-京万红软膏临床药物观察完成临床标本和血液采集及代谢组学等检测，明确中药-京万红软膏干预老年糖尿病多代谢紊乱引发的肢体血管病变的关键分子机制。 2、指导老年糖尿病多代谢紊乱引发的肢体血管病变诊断早及监测治疗等；揭示其药效基础和作用靶点；为构建老年糖尿病多代谢紊乱引发的肢体血管病变中医综合防控理论体系、明确中医药在老年糖尿病多代谢紊乱人群中的作用优势环节、形成老年糖尿病多代谢紊乱综合防治方案提供临床和基础数据。 3、提升老年糖尿病多代谢紊乱和肢体血管病变中医临床诊疗现有循证证据级别；建立结构化、全流程临床老年糖尿病多代谢紊乱引发的肢体血管病变中医诊疗病案数据库，提高老年糖尿病多代谢紊乱引发的肢体血管病变临床诊疗水平。  

Objectives of Study：

1. Establish and improve a multi-center clinical cohort for elderly patients with diabetes mellitus, multiple metabolic disorders, and limb vascular lesions; conduct a clinical drug observation of the traditional Chinese medicine - Jing Wan Hong Ointment, complete the collection of clinical specimens and blood, and perform metabolomics and other testing to clarify the key molecular mechanisms by which Jing Wan Hong Ointment intervenes in limb vascular lesions caused by multiple metabolic disorders in elderly diabetes.?? ?2. Guide the early diagnosis, monitoring, and treatment of limb vascular lesions caused by multiple metabolic disorders in elderly diabetes; reveal the pharmacodynamic basis and therapeutic targets of the intervention; provide clinical and fundamental data for constructing a comprehensive traditional Chinese medicine (TCM) theory system for the prevention and control of these lesions, for identifying the advantageous aspects of TCM in managing elderly populations with diabetes and multiple metabolic disorders, and for formulating a comprehensive prevention and treatment strategy for these conditions.?? ?3. Upgrade the current level of evidence-based medicine for the TCM clinical diagnosis and treatment of elderly diabetes with multiple metabolic disorders and limb vascular lesions; establish a structured, whole-process clinical medical record database for the TCM diagnosis and treatment of limb vascular lesions caused by multiple metabolic disorders in elderly diabetes, thereby enhancing the clinical standard of care for these conditions.?

纳入标准：

1.临床诊断符合西医2型糖尿病诊断标准。 2.静脉空腹血糖≤10mmol/L。 3.年龄65周岁（包括65岁）至80周岁（包括80周岁）,性别匹配。 4.溃疡深度至皮肤及皮下组织，未累及肌肉，肌腱和骨骼；无明显感染或经清创后感染已被控制。 5.清创后溃疡皮肤缺损面积在1-8cm^2； 6.尚未达到手术治疗的标准。 7.受试者同意，并签署知情同意书。

Inclusion criteria

1. Clinical diagnosis conforms to the Western medical diagnostic criteria for Type 2 Diabetes Mellitus. 2. Fasting venous blood glucose level is <= 10 mmol/L. 3. Age between 65 years (inclusive) and 80 years (inclusive); gender-matched. 4. Ulcer depth extends to the skin and subcutaneous tissue, without involvement of muscle, tendons, or bone; no significant infection present or infection is controlled after debridement. 5. Skin defect area of the ulcer is between 1 cm2 and 8 cm2 after debridement. 6. Does not meet the criteria requiring surgical intervention. 7. The subject agrees to participate and has signed the informed consent form.

排除标准：

1.既往有严重食物和（或）药物过敏史。 2.需外科手术治疗的糖尿病肢体血管病变。 3.合并有严重的心、脑、肝以及造血系统等疾病、或影响其生存的其它严重疾病。 4.电、化学、放射性等其他原因引起的皮肤溃疡及溃疡处有恶性病变；目标溃疡清创后＜1cm^2或＞8cm^2； 5.近3个月内接触过射线或使用过激素、化疗、生长因子类敷料及免疫 抑制剂； 6.溃疡侧肢体踝/肱动脉血压比值（ABI）＜0.6 或＞1.3； 7.目前溃疡创面存在显著感染者； 8.溃疡底有死骨及感染骨未经手术清除； 9.静脉曲张性溃疡； 10.血红蛋白＜90g/L；血小板＜100×10^9/L；血清白蛋白＜25g/L； 11.血压≥180/110mmHg； 12.妊娠或哺乳期； 13.严重心功能和肝功能异常（心功能NYHA分级≥3级，Child-Pugh C级）； 14.癌症患者；精神障碍或阿尔茨海默病患者； 15.近3个月内参与过临床药物试验或目前正在参加另外一项临床研究者。

Exclusion criteria：

1. History of severe food and/or drug allergies. 2. Diabetic limb vascular lesions requiring surgical treatment. 3. Comorbid with severe cardiac, cerebral, hepatic, or hematopoietic system diseases, or other severe diseases affecting survival. 4. Skin ulcers caused by electrical, chemical, radioactive, or other etiologies; OR presence of malignant changes at the ulcer site; OR the target ulcer area after debridement is < 1 cm2 or > 8 cm2. 5. Exposure to radiation or use of hormonal therapy, chemotherapy, growth factor-based dressings, or immunosuppressants within the past 3 months. 6. Ankle-Brachial Index (ABI) of the affected limb is < 0.6 or > 1.3. 7. Current presence of significant infection at the ulcer wound site. 8. Presence of necrotic bone or infected bone at the ulcer base that has not been surgically debrided. 9. Venous stasis ulcers (or Varicose ulcers). 10. Hemoglobin < 90 g/L; Platelet count < 100 × 10?/L; Serum albumin < 25 g/L. 11. Blood pressure >= 180/110 mmHg. 12. Pregnancy or lactation. 13. Severe cardiac or hepatic dysfunction (Cardiac function NYHA Class >= III, Child-Pugh Class C). 14. Patients with cancer; patients with psychiatric disorders or Alzheimer's disease. 15. Participation in another clinical drug trial within the past 3 months or current participation in any other clinical study.

采用分层区组随机化方法。分别按中心进行分层，选取合适段长。借助SAS统计软件PROC PLAN过程语句，给定种子数，产生268例受试者所接受处理（不同剂量组）的随机安排（即随机编码表）。

The stratified block randomization method was adopted. Stratification was conducted by center, and an appropriate block length was selected. With the help of the PROC PLAN statement in the SAS statistical software, a seed number was given to generate the random allocation (i.e., random coding table) of the treatments (different dose groups) received by 268 subjects.
