研究目的:

牙髓血运重建术作为重要的牙髓再生治疗方式,其疗效受多种因素影响且存在较大个体差异,但精准的个体化疗效预测工具仍缺乏。本研究旨在针对此临床需求,系统构建并严格验证基于多模态数据的牙髓血运重建术疗效智能预测模型。研究将整合患者临床指标、CBCT 等影像特征、根管菌群及人口学等多源异构数据,并科学定义含根尖愈合与牙根发育的复合疗效结局;运用先进特征工程与选择方法(如 LASSO)提取筛选关键预测因子,探索多模态数据融合策略以优化信息整合效能。在此基础上,构建并对比多种机器学习模型,通过严格的交叉验证与独立外部数据集验证,确保模型的泛化能力与临床稳健性。为提升临床应用价值,将采用 SHAP 等可解释性技术深入解析模型决策机制,明确影响疗效的核心驱动因素。本研究预期成果将牙髓血运重建术提供精准、可靠的个体化疗效预测手段,辅助临床医生制定更优治疗方案,对于推动该领域个体化精准医疗模式的发展、提升患者治疗成功率与远期生活质量具有重要的科学意义和临床应用前景。  

Objectives of Study:

As an important endodontic regenerative treatment, the efficacy of pulp revascularization is influenced by multiple factors and exhibits significant individual variability, yet accurate individualized efficacy prediction tools are still lacking. This study aims to address this clinical need by systematically constructing and rigorously validating an intelligent prediction model for pulp revascularization efficacy based on multimodal data. The study will integrate heterogeneous data from multiple sources, including patient clinical indicators, imaging features such as CBCT, root canal microbiota, and demographics, and scientifically define a composite efficacy outcome encompassing apical healing and root development. Advanced feature engineering and selection methods (such as LASSO) will be used to extract and screen key predictors, and multimodal data fusion strategies will be explored to optimize information integration efficiency. On this basis, multiple machine learning models will be constructed and compared, and rigorous cross-validation and independent external dataset verification will be used to ensure the generalization and clinical robustness of the models. To enhance their clinical application value, interpretability techniques such as SHAP will be used to deeply analyze the model's decision-making mechanism and identify the core drivers of efficacy. Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.经临床检查及影像学资料(根尖片、CBCT)明确诊断为年轻恒牙牙髓坏死(伴或不伴根尖周炎或根尖周脓肿),具有牙髓血运重建术的明确适应症; 2.于 2015 年 1 月 1 日至 2025 年 12 月期间,在浙江省口腔医院牙体牙髓科首次接受并完成规范化牙髓血运重建术治疗的患者; 3.牙齿状况:患牙为牙根未发育完成的年轻恒牙(Cvek 分期 1-4 期或 Nolla 分期 6-9 期); 4.资料完整性:术前具有完整的病史记录、清晰可评估的基线根尖片及符合研究分析质量要求的 CBCT 影像数据(DICOM 格式);术后具有至少 12 个月的定期、规范的临床及影像学随访记录,能够评估本研究设定的疗效结局(如根尖周愈合情况、牙根发育成熟度等);部分患者涉及根管内菌群数据,则需有术中按照标准化流程采集的根管内样本及合格的菌群检测数据;人口统计学信息及治疗相关关键指标记录完整。

Inclusion criteria

1. Clinical examination and imaging data (periapical radiographs, CBCT) confirmed the diagnosis of pulp necrosis in young permanent teeth (with or without apical periodontitis or periapical abscess), with clear indications for pulp revascularization therapy; 2. Patients who received and completed standardized pulp revascularization therapy in the Department of Conservative Dentistry and Endodontics at Zhejiang Provincial Stomatological Hospital for the first time between January 1, 2015, and December 2025; 3. Condition of the teeth: the affected teeth are young permanent teeth with incompletely developed roots (Cvek stages 1-4 or Nolla stages 6-9); 4. Data completeness: preoperative data must include complete medical history records, clear and assessable baseline periapical radiographs, and CBCT imaging data meeting research analysis quality requirements (DICOM format); postoperative data must include at least 12 months of regular, standardized clinical and imaging follow-up records, allowing evaluation of the efficacy outcomes set in this study (such as apical healing and root development maturation); for patients involving intracanal microbiota data, intraoperative root canal samples must be collected according to standardized procedures, and qualified microbiological test data must be available; demographic information and key treatment-related indicators must be fully recorded.

排除标准:

1.治疗不完整/不规范:未完成牙髓血运重建术全部治疗流程,或治疗方案严重偏离标准化操作指南者; 2.牙齿本身条件不佳:患牙存在无法进行充分冠方封闭的严重牙体组织缺损。患牙存在牙根内外吸收范围过大(如超过根长 1/3)、牙根穿孔、牙根纵裂、牙折累及根中 1/3 及以下等不适合行血运重建术的情况; 3.全身健康状况:患有严重影响免疫功能或组织愈合能力的全身系统性疾病(如未有效控制的糖尿病、血液系统疾病、恶性肿瘤正处于放化疗期、艾滋病等);长期服用可能显著影响牙髓或骨组织愈合的药物(如双膦酸盐、大剂量糖皮质激素、免疫抑制剂等); 4.资料不合格:术前或术后关键临床数据、影像资料(根尖片、CBCT)、菌群样本等严重缺失、伪影过重、质量低劣,无法满足本研究数据提取、量化及分析要求; 5.合并治疗干扰:在牙髓血运重建术治疗同期或随访期内,研究牙齿接受了其他可能显著干扰疗效独立评估的牙科治疗; 6.失访:无法取得至少 12 个月的有效随访数据者; 7.既往治疗史:研究牙齿曾接受过其他牙髓治疗(如部分牙髓切断术)。

Exclusion criteria:

1. Incomplete/irregular treatment: Failure to complete the entire treatment procedure of pulp revascularization, or treatment plans that significantly deviate from standardized guidelines; 2. Poor tooth condition: The affected tooth has severe loss of tooth structure that prevents adequate coronal sealing. The affected tooth has extensive internal or external root resorption (e.g., exceeding one-third of the root length), root perforation, longitudinal root fracture, or root fracture involving the middle third or less, making it unsuitable for revascularization; 3. General health conditions: Presence of systemic diseases that severely affect immune function or tissue healing (e.g., uncontrolled diabetes, hematologic disorders, malignant tumors undergoing radiotherapy or chemotherapy, AIDS, etc.); long-term use of drugs that may significantly affect pulp or bone healing (e.g., bisphosphonates, high-dose corticosteroids, immunosuppressants, etc.); 4. Inadequate data: Severe missing, heavily artifact-affected, or low-quality preoperative or postoperative key clinical data, imaging data (periapical radiographs, CBCT), microbiological samples, etc., making it impossible to meet the requirements for data extraction, quantification, and analysis in this study; 5. Concomitant treatment interference: During the pulp revascularization treatment period or follow-up, the study tooth received other dental treatments that could significantly interfere with independent evaluation of therapeutic efficacy; 6. Loss to follow-up: Unable to obtain at least 12 months of valid follow-up data; 7. Previous treatment history: The study tooth has undergone other pulp treatments (e.g., partial pulpotomy).
