研究目的:

1. 明确负重本体感觉训练与平衡装置训练相比对慢性功能性踝关节不稳的治疗效果。 2. 阐明负重本体感觉训练治疗功能性踝关节不稳患者的机制。  

Objectives of Study:

1. To clarify the therapeutic effects of weighted proprioceptive training compared to balance device training for chronic functional ankle instability. 2. To elucidate the mechanisms of weighted proprioceptive training in treating patients with functional ankle instability.

纳入标准:

1.过去3个月至1年内至少有一次急性踝关节扭伤; 2.多次出现踝关节“不稳定感”; 3.过去6个月内至少有两次踝关节不稳定或运动时的扭伤恐惧感; 4.坎伯兰踝关节不稳工具(CAIT)评分<=24分; 5.距骨倾斜试验和前抽屉试验阴性。

Inclusion criteria

1.At least one acute ankle sprain within the past 3 months to 1 year; 2. Multiple episodes of the feeling of ankle instability; 3.At least two episodes of ankle instability or fear of sprain during exercise in the past 6 months; 4.Cumberland Ankle Instability Tool (CAIT) score <=24; 5.Negative talus tilt and anterior drawer tests.

排除标准:

1.下肢手术史、骨折史或其他疾病引起的肌肉骨骼损伤史; 2.参与除本研究外的系统性运动训练; 3.任何可能影响本研究的已确诊临床疾病,如多发性硬化、类风湿性关节炎、2型糖尿病或心脏病。

Exclusion criteria:

1.History of lower limb surgery, fractures, or musculoskeletal injuries caused by other diseases; 2.Participation in systematic exercise training outside of this study; 3.Any diagnosed clinical diseases that may affect this study, such as multiple sclerosis, rheumatoid arthritis, type 2 diabetes, or heart disease.

受试者被要求在训练前进行热身,然后装配球型底踝关节平衡装置进行以下训练:1.双足站立、左右交替的单足站立、闭眼双足站立以及闭眼患侧腿单足站立;2.踏步和行走训练。专业的训练师会监督和调整练习内容以确保持续的挑战和进步。每周训练4次,共8周。

Participants were required to warm up before training and then fit the spherical balance device to perform the following exercises: 1. Standing on both feet, alternating single-leg standing on the left and right, standing on both feet with eyes closed, and single-leg standing on the affected side with eyes closed; 2. Stepping and walking exercises. A professional trainer would supervise and adjust the exercise content to ensure continuous challenge and progress. The training was conducted four times a week for a total of eight weeks.

受试者先进行10分钟热身,热身包括踝关节各个方向的运动,然后开始正式训练。受试者需要脱鞋后站在新装置上,根据受试者最大肌力的80%进行踝关节外翻肌向心、离心肌力训练,10次为一组,共进行3组。受试者站在新装置上,进行不同角度的踝关节内翻、外翻本体感觉训练,外翻角度为5°、20°,内翻角度为15°、30°,10次为一组,共进行2组。每周训练4次,共训练8周。

Intervention:

The subjects began with a 10-minute warm-up, which included ankle movements in all directions, followed by the formal training. The participants were required to remove their shoes and stand on the new device. They performed concentric and eccentric ankle eversion strength training at 80% of their maximum muscle strength, completing 3 sets of 10 repetitions each. While standing on the new device, they also performed proprioceptive training for inversion and eversion at different angles: eversion at 5° and 20°, and inversion at 15° and 30°, completing 2 sets of 10 repetitions each. The training was conducted 4 times per week for a total of 8 weeks.
