研究目的:

脑卒中后出现的膝过伸情况会致使患者平衡功能下降,步态呈现异常,并且显著增加跌倒风险,对患者的康复进程以及生活质量都产生了严重影响。当前临床干预多以开链运动为主,在改善功能性步态以及神经肌肉控制方面存在局限,本项目聚焦在探索本体感觉训练与闭链运动相结合对在脑卒中后患者出现的膝过伸情况所产生的康复治疗效果。  

Objectives of Study:

Post-stroke knee hyperextension can impair balance, cause abnormal gait patterns, and significantly increase fall risks, severely impacting both rehabilitation progress and quality of life. While current clinical interventions primarily focus on open-chain exercises, their effectiveness in improving functional gait and neuromuscular control remains limited. This study investigates the combined therapeutic effects of proprioceptive training and closed-chain exercises in addressing knee hyperextension in stroke patients.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.符合第4次全国脑血管疾病学术会议制订的脑卒中诊断标准,并经头颅CT或MRI诊断证实;2.下肢brunnstrom分期达到4期及以上;3.年龄40-70岁;4.生命体征稳定,意识清楚, 依从性高,可配合研究;5.Holden步行功能达到2级及以上;6.心肺功能好,能耐受康复训练。

Inclusion criteria

1. Meet the stroke diagnostic criteria established at the 4th National Cerebrovascular Disease Conference and confirmed by cranial CT or MRI; 2. Lower limb Brunnstrom stage 4 or higher; 3. Age 40-70 years; 4. Stable vital signs, clear consciousness, high compliance, and willingness to participate in the study; 5.Holden walking function grade 2 or higher; 6. Good cardiopulmonary function and ability to tolerate rehabilitation training.

排除标准:

1.骨关节疾病:如骨折术后、骨性关节炎;2.恶性进行性高血压;3.恶性肿瘤;4.有血栓形成;5.既往有精神疾病。

Exclusion criteria:

1. Bone and joint diseases: such as post-fracture surgery, osteoarthritis; 2. Malignant progressive hypertension; 3. Malignant tumors; 4. Thrombosis; 5. Previous mental illness.

Intervention:

Conduct an 8-week training program (5 sessions per week, 30 minutes each) combining: After routine rehabilitation exercises, perform closed-chain exercises combined with proprioception training. Closed-chain exercises include: seated-to-standing transfer micro-squatting, weight transfer training on balance pads or soft mats, modified bilateral bridge exercises with leg/patient-side dominance, and dynamic/static squatting exercises with leg/patient-side dominance. Proprioception training includes: balance training on support surfaces of varying firmness, joint position perception under closed-eye conditions, knee joint angle control training using biofeedback devices; dynamic balance training under interference conditions such as ball throwing and catching.exercises included: seated-to-standing transfer micro-squatting, weight transfer training on balance pads or soft mats, modified double-leg bridge with bilateral/affected-side dominance, and dynamic/static squats with bilateral/affected-side dominance. Proprioceptive training included: balance exercises on surfaces of varying firmness, joint position perception under closed-eye conditions, knee joint angle control training using biofeedback devices, and dynamic balance exercises under interference conditions such as ball throwing and catching.
