研究目的：

1.以ITHBC理论为研究框架，在明确老年T2DM口腔衰弱患者自我管理现状及其需求的基础上，结合德尔菲专家咨询法，开发一套科学、可行、针对性强的自我管理干预方案; 2.开展干预方案的应用研究，验证其在老年T2DM口腔衰弱患者中的实际效果与可行性，为推动老年T2DM口腔衰弱患者自我管理实践提供理论依据和实施参考，探索慢性病综合管理的新路径。  

Objectives of Study：

1. Based on the ITHBC theory as the research framework, develop a scientific, feasible, and highly targeted self-management intervention program by clarifying the current self-management status and needs of elderly patients with T2DM and oral frailty, and combining Delphi expert consultation method; 2. Conduct applied research on the intervention program to verify its practical effectiveness and feasibility in elderly patients with T2DM and oral frailty, providing theoretical support and implementation references for promoting self-management practices in these patients, and exploring new approaches to comprehensive chronic disease management.

纳入标准：

1.符合口腔衰弱人群筛查标准； 2.符合T2DM诊断的患者； 3.年龄>=60周岁； 4.自愿参与本研究，并签署知情同意书.

Inclusion criteria

1. Meet the screening criteria for individuals with oral frailty; 2. Patients diagnosed with T2DM; 3. Aged 60 years or older; 4. Voluntarily participate in this study and sign the informed consent form.

排除标准：

1.有认知障碍的患者； 2.正在参加口腔健康相关的其他干预性研究者.

Exclusion criteria：

1.Patients with cognitive impairments; 2.Researchers who are currently involved in other intervention studies related to oral health.
