研究目的：

基于角膜病大样本临床数据库、结构化病史信息及裂隙灯影像资料，并结合前期研发的便携式眼前节成像设备，在真实世界数据基础上构建多模态常见致盲性角膜病智能诊断模型，并探索其在便携终端上的迁移与应用，以期为基层及远程角膜病筛查和分级诊疗提供可行技术路径。  

Objectives of Study：

Based on real-world data—including large-sample clinical databases, structured medical histories, and slit-lamp images—and integrated with our proprietary portable anterior segment imaging device, we have developed a multi-modal AI diagnostic model for common blinding corneal diseases. By exploring the deployment of this model on portable terminals, we aim to offer a feasible technical solution for remote screening and tiered medical care at the primary level.

纳入标准：

1. 感染性角膜炎（符合下述任一诊断依据）： （1）细菌性角膜炎：角膜刮片涂片或培养阳性，或临床表现符合细菌性角膜炎特征（急性起病、剧烈疼痛、边界相对清晰的实质浸润、前房反应明显等），经角膜病专科医师综合判定； （2）真菌性角膜炎：刮片涂片见真菌丝/孢子或培养阳性，或裂隙灯示羽状浸润、卫星灶、免疫环等典型表现，经专科医师判定； （3）棘阿米巴性角膜炎：共焦显微镜发现典型棘阿米巴包囊/滋养体，或角膜刮片或培养阳性； （4）病毒性角膜炎（HSV/VZV）：HSV：典型树枝状/地图状上皮缺损或基质炎表现，必要时结合 PCR 检测结果；VZV：结合面部/眼睑疱疹病史及角膜受累表现； 2. 非感染性角膜炎： （1）免疫相关性角膜炎（如 Mooren 溃疡、边缘性角膜炎等），由角膜专科医师根据临床表现与全身情况综合诊断，排除感染证据；暴露性、神经营养性/麻痹性、丝状角膜炎等； （2）化学/热烧伤后非感染性角膜炎（明确排除继发感染）； 3．角膜营养不良：具备典型家族史或双眼对称性临床表现的上皮或基质营养不良，裂隙灯弥散光可清晰显示病变； 4. 角膜变性：如 Terrien 边缘性变性等具有典型裂隙灯特征的变性性角膜疾病初发病例。 其他一般条件：完整的初诊裂隙灯照相图像可获取，图像质量满足分析要求；临床病史记录相对完整，可提取核心问诊信息。

Inclusion criteria

1. Infectious keratitis (meeting any of the following diagnostic criteria): (1) Bacterial keratitis: Positive corneal scrapings smear or culture, or clinical manifestations consistent with bacterial keratitis characteristics (acute onset, severe pain, relatively well-defined stromal infiltrate, significant anterior chamber reaction, etc.), as comprehensively determined by a corneal specialist; (2) Fungal keratitis: Filaments/spores observed in scrapings smears or positive culture, or typical slit-lamp signs such as feathered infiltrates, satellite lesions, immune ring, etc., as determined by a specialist; (3) Acanthamoeba keratitis: Typical Acanthamoeba cysts/trophozoites found by confocal microscopy, or positive corneal scrapings or culture; (4) Viral keratitis (HSV/VZV): HSV: Typical dendritic/map-like epithelial defects or stromal keratitis, combined with PCR results if necessary; VZV: Corneal involvement in combination with a history of facial/eyelid herpes; 2. Non-infectious keratitis: (1) Immune-related keratitis (such as Mooren’s ulcer, marginal keratitis, etc.), diagnosed comprehensively by a corneal specialist based on clinical manifestations and systemic conditions, excluding evidence of infection; exposure keratitis, neurotrophic/paralytic keratitis, filamentary keratitis, etc.; (2) Non-infectious keratitis following chemical/thermal burns (with secondary infection clearly excluded); 3. Corneal dystrophy: Epithelial or stromal dystrophy with typical family history or bilateral symmetrical clinical manifestations, with slit-lamp diffuse illumination clearly showing the lesion; 4. Corneal degeneration: Primary cases of degenerative corneal diseases with typical slit-lamp features, such as Terrien’s marginal degeneration. Other general conditions: Complete slit-lamp photograph images of the initial visit are obtainable, and the image quality meets the analysis requirements; clinical history records are relatively complete, allowing extraction of core consultation information.

排除标准：

1.病因不明或临床资料不足以明确诊断的角膜病变； 2.明确混合感染（例如真菌+细菌）且难以区分主导病因的病例； 3.既往有严重眼表手术史导致局部解剖显著改变（如穿透性角膜移植、广泛结膜瓣覆盖、羊膜移植后瘢痕致使病灶无法评价等）； 4.就诊时角膜已穿孔、眼内容物脱出或继发眼内炎者； 5.裂隙灯照片质量严重不佳（明显失焦、曝光严重异常、眼脸遮挡主要病灶）影响评估者； 6.关键病史信息缺失，无法构建结构化病史问卷者； 7.非初诊病例（为避免治疗已显著改变病灶形态，仅纳入首次就诊时图像与病史)。

Exclusion criteria：

1. Corneal lesions with unknown causes or insufficient clinical information to make a clear diagnosis; 2. Confirmed mixed infections (e.g., fungal and bacterial) where it is difficult to distinguish the primary cause; 3. History of significant ocular surface surgery causing notable local anatomical changes (such as penetrating keratoplasty, extensive conjunctival flap coverage, or scarring after amniotic membrane transplantation that makes the lesion unassessable); 4. Corneal perforation, prolapse of intraocular contents, or secondary endophthalmitis at the time of visit; 5. Poor-quality slit-lamp photographs (evident out-of-focus, severe exposure issues, eyelid obstructing the main lesion) affecting assessment; 6. Missing key medical history information making it impossible to construct a structured history questionnaire; 7. Non-initial cases (to avoid lesions that have been significantly altered by treatment, only images and medical history from the first visit are included).

感染性角膜炎以病原学检查为金标准，结合高年资医生评估的临床典型的症状和体征、对治疗的反应等综合临床诊断，当病原学结果阴性但临床表现典型且治疗有效时，以至少2名副主任医师及以上级别专家的共识诊断为最终参照标准。非感染性角膜炎、角膜营养不良、角膜变性以高年资医生基于临床症状体征、对治疗的反应为金标准

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

Infectious keratitis is diagnosed using pathogen examination as the gold standard, combined with the comprehensive clinical diagnosis based on typical symptoms and signs assessed by experienced doctors, as well as responses to treatment. When pathogen test results are negative but the clinical presentation is typical and treatment is effective, the final reference standard is a consensus diagnosis by at least two associate chief physicians or higher-level experts. For non-infectious keratitis, corneal dystrophy, and corneal degeneration, the gold standard is the evaluation by experienced doctors based on clinical symptoms, signs, and treatment responses.

容易混淆的疾病人群（即与目标疾病不易区分的一种或多种不同疾病，应避免采用正常人群对照的病例-对照设计）：

容易混淆的疾病人群主要集中在三大方面：感染与非感染/免疫性角膜炎，需鉴别周边溃疡性角膜炎（PUK）与非典型感染，以及神经营养性/暴露性角膜炎的继发感染。角膜营养不良（遗传）与角膜变性（后天）的表现有重叠，需仔细区分。此外，严重眼表疾病（如干眼、SJS）引起的晚期瘢痕或继发病变，可能与研究目标疾病的终末期相似。核心挑战在于区分原发病因是感染、自身免疫还是结构失调。
