研究目的：

种植修复已成为缺失牙修复的主要手段，对于骨量不足的种植位点，引导骨再生（Guided Bone Regeneration, GBR）是处理骨缺损的重要方法。在GBR手术操作过程中，是否需要在受体床进行去皮质（decortication）一直存在争议。研究旨在明确去皮质操作在下颌后牙区等高密度皮质骨区域是否真正带来额外获益，为临床医生提供循证依据，帮助优化GBR手术流程，避免不必要的创伤和风险。  

Objectives of Study：

Dental implant–supported restoration has become the primary treatment modality for the rehabilitation of missing teeth. For implant sites with insufficient bone volume, guided bone regeneration (GBR) is a well-established approach for managing alveolar bone defects. During GBR procedures, whether decortication of the recipient bed is necessary remains controversial. This study aims to clarify whether decortication provides additional clinical benefits in high-density cortical bone regions, such as the posterior mandible, thereby providing evidence-based guidance for clinicians to optimize GBR protocols and to avoid unnecessary surgical trauma and potential risks.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 拔牙后3个月以上在后牙区进行种植体植入； 2. 种植体植入后存在颊侧骨缺损，但缺损范围小于种植体长度的50%（根据Terheyden的分类为1/4型缺损）（Cordaro 等，2019）； 3. 患者全身健康或全身状况稳定，可耐受常规口腔种植手术； 4. 患者在2020年7月至2023年7月间入组； 5. 年龄≥18岁。

Inclusion criteria

1. Implant placement performed in the posterior region at least 3 months after tooth extraction; 2. Presence of a buccal bone defect following implant placement, with the defect extent involving less than 50% of the implant length (classified as a Class 1/4 defect according to Terheyden’s classification) (Cordaro et al., 2019); 3. Patients who are systemically healthy or have stable systemic conditions and are able to tolerate routine oral implant surgery; 4. Patients enrolled between July 2020 and July 2023; 5. Age >= 18 years.

排除标准：

1. 严重全身性疾病（如未控制的糖尿病、免疫缺陷等） 2. 长期使用影响骨代谢或愈合的药物（如糖皮质激素） 3. 重度吸烟（＞10-20支/天）或长期酗酒 4. 口腔卫生状况不良或未控制的牙周炎 5. 妊娠期或既往头颈部放疗史

Exclusion criteria：

1. Severe systemic diseases (e.g., uncontrolled diabetes mellitus, immunodeficiency, etc.) 2. Long-term use of medications that may affect bone metabolism or healing (e.g., corticosteroids) 3. Heavy smoking (>10–20 cigarettes/day) or chronic alcohol abuse 4. Poor oral hygiene or uncontrolled periodontitis 5. Pregnancy or a history of head and neck radiotherapy

本研究基于华西口腔医院种植科病历管理系统提取相关临床资料，采用统一设计的病例记录表（CRF）进行数据采集，并录入电子数据库进行管理。所有数据在分析前均进行匿名化处理，仅限授权研究人员访问。

Data were collected using standardized CRFs, with clinical information retrieved from the medical record management system of the Department of Oral Implantology, West China Hospital of Stomatology, which served as the electronic data source.
