研究目的:

甲状腺乳头状癌(PTC)颈部淋巴结转移(LNM)风险评估仍缺乏安全、精准的预测手段,是导致PTC过度治疗的重要原因。针对PTC患者LNM风险评估难和人工智能模型可解释性不足两大问题,本研究拟基于常规临床检查阶段的超声检查数据,构建一个融合临床信息与多模态超声图像的可解释性人工智能模型,以实现对PTC患者LNM风险的无创精准预测,为个体化治疗提供决策支持。  

Objectives of Study:

Accurate preoperative assessment of cervical lymph node metastasis (LNM) in papillary thyroid carcinoma (PTC) remains challenging and is a major cause of overtreatment. To address the limitations of current LNM risk evaluation and the low interpretability of artificial intelligence (AI) models, this study proposes an explainable AI framework integrating clinical data and multimodal ultrasound imaging acquired during routine examinations. The model aims to achieve non-invasive and precise prediction of LNM risk in PTC patients, providing a reliable basis for individualized clinical decision-making.

纳入标准:

①年龄均在18岁及以上;②患者均接受甲状腺切除术及淋巴结清扫术,并有对应的病理结果;③术前均进行甲状腺结节的超声检查。

Inclusion criteria

1.Age >=18 years;2.Underwent thyroidectomy with lymph node dissection, with corresponding histopathologic confirmation;3.Underwent preoperative ultrasonography for evaluation of thyroid nodules.

排除标准:

1.颈部外伤史;2.术前接受过PTC的相关治疗;3.术前超声图像质量不佳;4.资料不完整的患者。

Exclusion criteria:

1.History of cervical trauma; 2.Prior therapy for papillary thyroid carcinoma before surgery; 3.Poor-quality preoperative ultrasound images; 4.Incomplete clinical or imaging data;

Postoperative resected specimens will be examined according to routine histopathological procedures and a pathology report will be issued. The histopathological diagnosis will serve as the reference-standard outcome, with documentation of histologic type and lymph node metastasis status; if available, capsular invasion and extrathyroidal extension will also be recorded.

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data collection and management consist of a standardized Case Record Form (CRF) and an Electronic Data Capture (EDC) system. Clinical variables, laboratory results, and multimodal ultrasound images are recorded according to the predefined CRF and imported from the hospital EMR and PACS systems. All data are stored on secured servers with access control, routine quality checks, and regular backups to ensure data accuracy, completeness, and integrity.
