研究目的:

主要目的: 1.验证各项自动化算法在识别颅内脑电中病理性高频振荡的准确性,并断评估其在精确定位癫痫灶核心区方面的临床辅助价值; 2. 探究并对比算法与人工识别颅内脑电高频振荡的识别差异与有效性。 次要目的: 1.研究颅内脑电的癫痫发作间期病理性与任务态下生理性高频振荡的特征差异,辅助临床对高频振荡识别的精准判断; 2.以算法生成的病理性高频振荡 “灶点” 的切除率与患者术后发作状态的相关性。  

Objectives of Study:

Primary Objectives: 1. To validate the accuracy of various automated algorithms in detecting pathological high-frequency oscillations in intracranial EEG and to preliminarily assess their clinical utility in precisely localizing the epileptogenic core region; 2. To explore and compare the differences and effectiveness between algorithm-based and manual identification of high-frequency oscillations in intracranial EEG. Secondary Objectives: 1. To study the characteristic differences between pathological high-frequency oscillations during the interictal period and physiological high-frequency oscillations during task states in intracranial EEG, aiding clinicians in accurately identifying high-frequency oscillations; 2. To examine the correlation between the resection rate of pathological high-frequency oscillation 'foci' generated by the algorithm and the postoperative seizure outcomes of patients.

纳入标准:

1.药物难治性癫痫患者; 2.经头皮录像脑电图、神经影像学、神经心理评估等检查评估后,实施外科手术,或者实施立体定向深部电极置入,或者硬膜下电极置入的患者; 3.无严重智力障碍正常,能够配合完成实验任务,若 MoCA 评分小于 26 分则不参与研究; 4.受试者或受试者监护人同意加入并签署同意书。

Inclusion criteria

1. Patients with drug-resistant epilepsy; 2. Patients who, after evaluation by scalp video EEG, neuroimaging, neuropsychological assessment, etc., undergo surgical procedures, stereotactic deep electrode implantation, or subdural electrode implantation; 3. Patients without severe intellectual disabilities, able to cooperate and complete experimental tasks; those with a MoCA score below 26 are not to participate in the study; 4. The subject or the subject's guardian agrees to participate and signs the consent form.

排除标准:

1.存在严重视力或听力障碍,由于其他器质性问题而无法正常理解语言;存在严重视力或听力障碍,由于其他器质性问题而无法正常理解语言;主试会询问患者或其家属是否有意愿参与此项目。根据认知实验任务的难度和患者的认知能力、发育状态、运动状态、情绪状态等进行综合评估。若 MoCA 评分小于 26 分则不参与研究; 2.对照组存在严重基础疾病或除癫痫外其他严重神经系统疾病。

Exclusion criteria:

1. There are severe visual or hearing impairments, and due to other organic issues, the person cannot normally understand language; the examiner will ask the patient or their family whether they are willing to participate in this project. A comprehensive assessment will be conducted based on the difficulty of the cognitive experimental tasks and the patient's cognitive ability, developmental status, motor function, emotional state, etc. If the MoCA score is less than 26, the individual will not participate in the study; 2. The control group has severe underlying diseases or other serious neurological disorders besides epilepsy.
