研究目的：

本研究针对携带 GBA 基因变异的帕金森病（GBA-PD）患者，结合横断面与纵向随访数据，以基因检测阴性帕金森病（GU-PD）患者为参照，系统分析其临床特征、疾病进展及自然病史，此外评估不同 GBA 变异位点对发病风险、临床表型及进展速率的影响，旨在为帕金森病的早期诊断、遗传筛查及精准治疗提供科学依据。同时，基于 [18]F-FDG PET/CT 在揭示脑代谢异常方面的重要性，比较 GBA-PD 与 GU-PD 患者的脑代谢特征，探讨代谢差异与疾病演变的关系，为预后评估提供影像学支持。  

Objectives of Study：

This study focuses on Parkinson’s disease patients carrying GBA gene variants (GBA-PD). By integrating cross-sectional and longitudinal follow-up data, and using genetically undefined Parkinson’s disease (GU-PD) patients as a reference group, we aim to systematically analyze the clinical characteristics, disease progression, and natural history of GBA-PD. In addition, we will evaluate the effects of different GBA variant sites on disease risk, clinical phenotype, and progression rate, providing scientific evidence for the early diagnosis, genetic screening, and precision treatment of Parkinson’s disease.Furthermore, considering the importance of [^18F]FDG PET/CT in revealing cerebral metabolic abnormalities, this study will compare the brain metabolic patterns between GBA-PD and GU-PD patients, explore the relationship between metabolic differences and disease evolution, and provide imaging-based support for prognostic assessment.

纳入标准：

1 本研究GBA-PD组纳入标准如下： （1）临床确诊帕金森病：2012 年 2 月至 2015 年 12 月根据英国伦敦帕金森协会脑库(United Kingdom Parkinson's Disease Society Brain Bank)制定的诊断标准，2015 年 12 月至 2025 年 3 月根据运动障碍协会（Movement Disorder Society, MDS）制定的诊断标准进行 PD 诊断。年龄在 18-90 岁之间，性别不限。所有诊断均由至少三名神经内科运动障碍亚专科专家共同确认。 （2）已行帕金森病相关二代测序及多重连接依赖式探针扩增检测，并经过长读长PCR验证结果为GBA基因杂合突变的患者。 2 本研究GU-PD组患者纳入标准如下： （1）临床确诊帕金森病：2012 年 2 月至 2015 年 12 月根据英国伦敦帕金森协会脑库(United Kingdom Parkinson's Disease Society Brain Bank)制定的诊断标准，2015 年 12 月至 2025 年 3 月根据运动障碍协会（Movement Disorder Society, MDS）制定的诊断标准进行 PD 诊断。年龄在 18-90 岁之间，性别不限。所有诊断均由至少三名神经内科运动障碍亚专科专家共同确认。 （2）已行帕金森病相关二代测序及多重连接依赖式探针扩增检测，未检测出已知的致病突变和风险位点变异的患者。

Inclusion criteria

Inclusion Criteria for the GBA-PD Group 1.Clinically diagnosed Parkinson’s disease:From February 2012 to December 2015, diagnoses were made according to the United Kingdom Parkinson’s Disease Society Brain Bank diagnostic criteria.From December 2015 to March 2025, diagnoses were made based on the Movement Disorder Society (MDS) clinical diagnostic criteria for Parkinson’s disease.Participants are 18 to 90 years old, with no restriction on sex. All diagnoses were independently confirmed by at least three neurologists specialized in movement disorders. 2.Genetic testing:Patients who have undergone next-generation sequencing (NGS) and multiplex ligation-dependent probe amplification (MLPA) for Parkinson’s disease–related genes, and whose results were validated by long-range PCR, confirming a heterozygous GBA gene mutation. Inclusion Criteria for the GU-PD Group 1.Clinically diagnosed Parkinson’s disease:From February 2012 to December 2015, diagnoses were made according to the United Kingdom Parkinson’s Disease Society Brain Bank diagnostic criteria.From December 2015 to March 2025, diagnoses were made based on the Movement Disorder Society (MDS) clinical diagnostic criteria for Parkinson’s disease.Participants are 18 to 90 years old, with no restriction on sex. All diagnoses were independently confirmed by at least three neurologists specialized in movement disorders. 2.Genetic testing:Patients who have undergone next-generation sequencing (NGS) and multiplex ligation-dependent probe amplification (MLPA) for Parkinson’s disease–related genes, and no known pathogenic mutations or risk variants were detected.

排除标准：

本研究排除标准如下： （1）已行脑深部电刺激手术的帕金森病患者。 （2）帕金森病叠加综合征。 （3）其他继发性帕金森病。 （4）合并其他中枢或者周围神经系统疾病者。 （5）合并其他严重内科系统疾病，无法耐受评估者。 （6）携带其他已知的致病突变和风险位点变异。

Exclusion criteria：

Exclusion Criteria 1.Patients with Parkinson’s disease who have undergone deep brain stimulation (DBS) surgery. 2.Patients with Parkinson-plus syndromes. 3.Patients with secondary (symptomatic) Parkinsonism. 4.Patients with other central or peripheral nervous system disorders. 5.Patients with severe systemic medical conditions who are unable to tolerate clinical assessments. 6.Patients carrying other known pathogenic mutations or risk variants.
