研究目的：

（1）构建针对老年心力衰竭患者衰弱的多维干预方案 （2）应用干预方案，评估多维干预方案对老年心力衰竭患者衰弱状态的影响  

Objectives of Study：

(1) Develop a multidimensional intervention program targeting frailty in elderly patients with heart failure (2) Implement the intervention program and evaluate its impact on frailty status in elderly patients with heart failure

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥60岁；（2）符合《中国心力衰竭诊断和治疗指南2024》的诊断标准且纽约心脏病协会（NYHA）心功能分级为I-III级的稳定期HF患者；（3）衰弱标准：FFP得分＞2分；（4）意识清楚，能与之进行沟通交流。（5）自愿签署知情同意书。

Inclusion criteria

(1) Age >= 60 years; (2) Patients with stable heart failure (HF) meeting the diagnostic criteria of the Chinese Guidelines for Diagnosis and Treatment of Heart Failure 2024 and classified as New York Heart Association (NYHA) functional class I-III; (3) Frailty criteria: Frailty Frailty Frailty Frailty (FFP) score > 2 points; (4) Alert and able to communicate effectively. (5) Voluntarily sign the informed consent form.

排除标准：

（1）合并重大躯体疾病；（2）因腰椎间盘突出、骨折、偏瘫等导致肢体功能明显受限；（3）服用可造成出现类似衰弱状态的药物（如抗抑郁药、左旋多巴、盐酸多奈哌齐）；（4）参与其他临床干预性研究的患者。

Exclusion criteria：

(1) Patients with significant concomitant physical illnesses; (2) Patients with significant functional limitations due to conditions such as lumbar disc herniation, fractures, or hemiplegia; (3) Patients taking medications that may cause a state of apparent debility (e.g., antidepressants, levodopa, donepezil hydrochloride); (4) Patients participating in other clinical interventional studies.

干预组在对照组健康教育的基础上执行为期12周的多维干预。生理维度以抗阻训练与个体化营养指导为主；心理维度以认知训练为核心；社会维度以护士主导小组支持及家庭同伴交流为主要形式。

健康教育与院外随访：住院期间为患者讲解HF及衰弱相关知识、运动获益与重要性，鼓励运动；出院前完成药物、饮食及睡眠注意事项的个体化宣教。出院后每两周电话随访一次，督促患者按医嘱门诊复查，并通过门诊或线上平台收集随访数据。

Health Education and Post-Discharge Follow-Up: During hospitalization, educate patients on heart failure (HF) and frailty-related knowledge, the benefits and importance of exercise, and encourage physical activity. Prior to discharge, provide individualized instruction on medication, dietary, and sleep precautions. Conduct biweekly telephone follow-ups post-discharge to ensure patients attend outpatient follow-up appointments as prescribed, and collect follow-up data through outpatient visits or online platforms.
