研究目的：

热液烫伤是儿童期常见的意外伤害之一，可导致皮肤局部损伤，进而可迅速引起全身各系统一系列的病变，严重时可危及生命。在对烫伤愈合所需时间以及最终瘢痕形成的预估中，烫伤深度尤其重要。烧伤创面的愈合过程复杂，在其愈合过程，需要细胞之间、细胞与细胞外基质及细胞因子间相互影响和相互协同。自体富血小板血浆（platelet-rich plasma，PRP）是从自体血中提取出来的血小板浓缩物，含有多种生物活性物质，具有促进细胞增殖、分化、基质合成、骨或软组织再生与修复以及抗炎等作用。PRP凝胶可能通过在前期提高TGF-β1表达，促进创面愈合，治疗后期表达的下降可使瘢痕组织形成风险下降。湿性愈合方式可以调节创面氧张力，促进毛细血管的形成；有利于坏死组织和纤维蛋白的溶解；促进多种生长因子释放，在创面愈合过程中起着非常重要的作用；保持创面恒温，利于组织生长，无结痂形成，避免新生肉芽组织的再次机械性损伤；保护创面神经末梢，减轻疼痛。使得湿性愈合速度约为传统愈合的2倍。因此，PRP联合湿性愈合方式有望可进一步促进创面愈合速度，减轻创面疼痛，减少创伤后瘢痕形成。  

Objectives of Study：

Scalds are one of the common accidental injuries in childhood. They can cause localized skin damage, which may rapidly lead to a series of systemic pathological changes and, in severe cases, can be life-threatening. When estimating the time required for burn healing and the formation of the final scar, the depth of the burn is particularly important. The healing process of burn wounds is complex, involving interactions and coordination between cells, between cells and the extracellular matrix, and among cytokines. Autologous platelet-rich plasma (PRP) is a platelet concentrate extracted from a person's own blood, containing various bioactive substances that promote cell proliferation, differentiation, matrix synthesis, bone or soft tissue regeneration and repair, and anti-inflammatory effects. PRP gel may promote wound healing by increasing TGF-β1 expression in the early stages, and the subsequent decrease in expression during later stages may reduce the risk of scar formation. The wet healing method can regulate wound oxygen tension and promote capillary formation; it helps dissolve necrotic tissue and fibrin; it promotes the release of multiple growth factors, playing a crucial role in the wound healing process; it maintains a constant wound temperature, supports tissue growth, prevents scab formation, and avoids repeated mechanical damage to newly formed granulation tissue; it also protects the nerve endings in the wound, reducing pain. Wet healing can achieve a healing rate approximately twice that of traditional methods. Therefore, the combination of PRP and wet healing is expected to further accelerate wound healing, relieve wound pain, and reduce post-injury scar formation.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.患儿均因烫伤且面积≥5%，伤后第 3 天评估创面深度达深 II°； 2.创面位置为首次损伤，无疤痕、胎记、血管瘤等影响肤色病灶或印记； 3.患儿年龄在 1-6 岁，无合并其他脏器损伤，如呼吸道吸入性损伤，无风湿免疫性疾病、皮肤疾病、营养缺陷等疾病； 4.家属接受治疗方案且患儿耐受治疗。

Inclusion criteria

1. The children all suffered burns with an area >=5%, and on the 3rd day after injury, the wound depth was assessed as deep II°; 2. The wound is at the site of the initial injury, without scars, birthmarks, hemangiomas, or other lesions affecting skin color or marks; 3. The children are aged 1-6 years, with no coexisting organ injuries, such as inhalation injuries to the respiratory tract, and no rheumatic autoimmune diseases, skin diseases, nutritional deficiencies, or other conditions; 4. The family accepts the treatment plan, and the children tolerate the treatment.

排除标准：

1.创面在治疗过程已明确出现植皮标准情况； 2.治疗过程新发现患儿有原发皮肤疾病、风湿免疫性疾病或其他家属隐瞒、不知情疾病等； 3.治疗过程患者出现对敷料、富血小板血浆等过敏反应; 4.家属不接受该治疗方案或不同意研究组采集数据。

Exclusion criteria：

1. The wound has clearly met the criteria for skin grafting during the treatment process; 2. During the treatment, it is newly discovered that the patient has a primary skin disease, rheumatic immune disease, or other conditions that are unknown or concealed by the family; 3. During the treatment, the patient develops an allergic reaction to dressings, platelet-rich plasma, etc.; 4. The family does not accept this treatment plan or does not agree to the collection of data by the research group.
