研究目的：

通过回顾性调查Alfalfa APP的临床应用情况，对患者亲属参与Alfalfa APP的远程华法林管理的安全性与有效性进行评价，与常规的患者本人基于应用程序的远程华法林自我管理是否有差异，对患者使用Alfalfa APP的不良反应、危险因素等进行相关分析，同时构建患者线上华法林管理的血栓和出血风险评估模型，以期促进Alfalfa APP的临床应用，提高患者用药依从性和疗效，降低不良反应, 对保障今后相关患者亲属在使用Alfalfa APP的安全性和有效性有重要意义。  

Objectives of Study：

By retrospectively investigating the clinical application of Alfalfa APP, evaluating the safety and efficacy of remote warfarin management by patients' relatives participating in Alfalfa APP, whether there is any difference with the conventional remote warfarin self-management based on the app by patients themselves. We also analyzed the adverse reactions and risk factors of patients using Alfalfa APP, and constructed a thrombosis and bleeding risk assessment model for online warfarin management, in order to promote the clinical application of Alfalfa APP, to improve patients' medication adherence and efficacy, and to reduce the adverse reactions, which is of great significance to guarantee the safety and efficacy of Alfalfa APP for relatives of the patients concerned in the future.

纳入标准：

1.注册Alfalfa APP的患者；
2.患者基本病历信息及诊疗过程详细；
3.用药目的、用法用量及用药疗程明确。

Inclusion criteria

1.Patients enrolled in the Alfalfa APP;
2.Detailed information on the patient's basic medical history and treatment process;
3.Purpose of medication, dosage and medication regimen are clear.

排除标准：

1.患者用药信息不全；
2.无进行过自我上传报告的患者；

Exclusion criteria：

1.Incomplete information on patient medication;
2.Patients who have not performed a self-upload report;
