研究目的：

本研究为实效性随机对照试验，旨在探究地舒单抗对男性骨质疏松症或骨量减少患者糖脂-心血管-骨骼代谢的影响。  

Objectives of Study：

This study is a pragmatic randomized controlled trial, aiming to investigate the glucolipid-cardiometabolic-osteo effects of denosumab on male osteoporosis.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1: 具有髋部或椎体脆性骨折史（无发生时间限制）的40岁及以上成年男性； 2: 患者签署知情同意书； 3: 50岁以下男性，全髋、股骨颈或腰椎（L1-4）骨密度Z值≤-2.0； 4: 50岁及以上男性，全髋、股骨颈或腰椎（L1-4）骨密度T值≤-1.0。

Inclusion criteria

1. Adult males aged 40 years and older with a history of fragility fracture of the hip or vertebral body (with no time limit for occurrence); 2. The patient has signed the informed consent form; 3. Males under 50 years of age with a bone mineral density (BMD) Z-score <= -2.0 at the total hip, femoral neck, or lumbar spine (L1-4); 4. Males aged 50 years and older with a bone mineral density (BMD) T-score <= -1.0 at the total hip, femoral neck, or lumbar spine (L1-4).

排除标准：

1: 诊断其他代谢性骨病，如甲状旁腺功能亢进、畸形性骨炎、骨软化症、成骨不全症等； 2: 严重肝肾功能不全：血清碱性磷酸酶水平高于正常上限的1.5倍，或谷丙氨酸转氨酶大于正常上限的3倍，或计算的肌酐清除率<35.0ml/min/1.73m^2； 3: 罹患1型糖尿病、使用胰岛素血糖仍控制不佳（糖化血红蛋白>7.5%）的2型糖尿病； 4: 罹患家族性高甘油三酯血症、家族性高胆固醇血症等遗传性脂质代谢异常性疾病； 5: 罹患类风湿性关节炎、系统性红斑狼疮等结缔组织病； 6: 罹患转移性实体肿瘤或血液系统恶性肿瘤； 7: 地舒单抗禁忌证：低钙血症、对地舒单抗活性成份或任何辅料成份过敏； 8: 1年内接受过地舒单抗、双膦酸盐、特立帕肽或罗莫珠单抗治疗；6月内接受糖皮质激素治疗超过3月以上者；6月内使用过噻唑烷二酮类、维生素K2、降钙素、合成代谢类固醇、生长激素者； 9: 外力引起的暴力性骨折。

Exclusion criteria：

1. Diagnosis of other metabolic bone diseases, such as hyperparathyroidism, Paget's disease of bone, osteomalacia, osteogenesis imperfecta, etc. 2. Severe hepatic or renal insufficiency: serum alkaline phosphatase level higher than 1.5 times the upper limit of normal, or alanine aminotransferase greater than 3 times the upper limit of normal, or calculated creatinine clearance rate < 35.0 ml/min/1.73m2; 3. Type 1 diabetes mellitus, or type 2 diabetes mellitus with poorly controlled blood glucose despite insulin use (glycated hemoglobin > 7.5%); 4. Hereditary lipid metabolism disorders such as familial hypertriglyceridemia and familial hypercholesterolemia; 5. Connective tissue diseases such as rheumatoid arthritis and systemic lupus erythematosus; 6. Metastatic solid tumors or hematological malignancies; 7. Contraindications to denosumab: hypocalcemia, hypersensitivity to the active ingredient of denosumab or any of its excipients; 8. Receipt of denosumab, bisphosphonates, teriparatide, or romosozumab within 1 year; receipt of glucocorticoid therapy for more than 3 months within 6 months; use of thiazolidinediones, vitamin K2, calcitonin, anabolic steroids, or growth hormone within 6 months; 9. Traumatic fractures caused by external force.
