研究目的：

中枢损伤（脑卒中、脑外伤、脑瘫等）慢性期遗留的言语认知障碍进一步改善难度极大，前期在颈7神经根切断术（NC7）治疗失语患者和左右颈7交叉移位术治（CC7）治疗中枢性偏瘫患者研究中观察到患者出现显著言语认知功能改善。研究者拟通过本个案研究，探索通过周围神经刺激调控和颈7神经根切断手术（NC7）诱发脑重塑，从而增强言语认知功能的恢复，并探索其机制，为恢复中枢神经损伤慢性期的言语认知障碍提供新的治疗思路和方法。  

Objectives of Study：

Improving chronic speech and cognitive deficits after central nervous system injury (e.g., from stroke, brain trauma, or cerebral palsy) is exceptionally difficult. Serendipitously, our prior research into C7 nerve root transection (NC7) for aphasia and contralateral C7 cross-transfer (CC7) for central hemiplegia revealed notable patient improvements in these domains. Building upon this finding, the present case study will explore a combined approach of peripheral nerve neuromodulation and C7 transection to promote brain remodeling. This research seeks to enhance functional recovery of speech and cognition and to elucidate the mechanisms involved, offering a potential new direction for treating these persistent impairments.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄14-80岁，性别不限； 2.经神经内科确诊为脑损伤后认知障碍或脑血管性认知障碍患者，符合脑损伤后认知障碍或脑血管性认知障碍临床表现及影像学特点； 3.MoCA-Basic评分＜24分； 4.起病时间超过6个月； 5.具有24h监护的稳定陪伴者； 6.患者及监护人签署知情同意书.

Inclusion criteria

1. Age 14-80 years, any gender; 2. Diagnosed by a neurologist with post-brain injury cognitive impairment or vascular cognitive impairment, meeting the clinical manifestations and imaging characteristics of post-brain injury cognitive impairment or vascular cognitive impairment; 3. MoCA-Basic score <24; 4. Onset time exceeding 6 months; 5. Has a stable caregiver available 24 hours a day; 6. Patient and caregiver have signed the informed consent form.

排除标准：

1.存在其他非脑血管疾病或脑外伤导致的认知障碍，如阿尔茨海默病、额颞叶痴呆、路易体痴呆等； 2.存在其他影响认知功能的颅内或颅外病变，如脑肿瘤、严重脑积水；甲状腺功能减退等； 3.严重系统性疾病，如恶性肿瘤、肝肾功能不全、凝血功能障碍、传染性疾病等； 4.患者及陪伴者无法配合治疗者； 5. 3 个月内参加过其他临床干预性试验者。

Exclusion criteria：

1. Presence of cognitive impairment caused by other non-cerebrovascular diseases or brain injuries, such as Alzheimer's disease, frontotemporal dementia, or Lewy body dementia; 2. Presence of other intracranial or extracranial lesions affecting cognitive function, such as brain tumors, severe hydrocephalus, or hypothyroidism; 3. Severe systemic diseases, such as malignant tumors, liver or kidney dysfunction, coagulopathy, or infectious diseases; 4. Patients and caregivers unable to cooperate with treatment; 5. Participation in other clinical interventional trials within the past 3 months.
