研究目的：

1 描述我国骨科非伤口专科护士参与骨科手术伤口管理现状，以及其对手术伤口护理的知识、态度、行为和培训需求；了解骨科非伤口专科护士视角下外科护士实施手术伤口管理实践的内容和要素，为方案构建提供借鉴。 2 以循证实践方法学为指导，综合最佳证据，为构建适合我国临床情境的护士主导骨科I类手术伤口管理方案提供文献支持。 3 聚焦临床情境与利益相关人群需求，探索将经方案汇总的证据引入临床实践的多维度障碍和促进因素，以构建护士主导骨科I类手术伤口管理实施方案。 4 评价护士主导骨科I类手术伤口管理方案的应用对护士循证实践行为、手术伤口管理实践的知信行、组织结构层面的影响。  

Objectives of Study：

1.To describe the current status of orthopedic non–wound-specialist nurses’ involvement in surgical wound management in China, and to assess their knowledge, attitudes, behaviors, and training needs regarding surgical wound care; to explore the content and key elements of surgical wound management practices implemented by surgical nurses from the perspective of orthopedic non–wound-specialist nurses, thereby providing insights for program development. 2.To provide literature support for the development of a nurse-led surgical wound management program for Class I orthopedic surgeries adapted to the Chinese clinical context, guided by the methodology of evidence-based practice and integrating the best available evidence. 3.To focus on clinical contexts and stakeholder needs, and to identify the multidimensional barriers and facilitators influencing the integration of evidence-based interventions summarized in the program into clinical practice, so as to develop a nurse-led implementation plan for orthopedic Class I surgical wound management. 4.To evaluate the impact of implementing the nurse-led orthopedic Class I surgical wound management program on nurses’ evidence-based practice behaviors, their knowledge, attitudes, and practices (KAP) related to surgical wound management, as well as on organizational and structural outcomes.

纳入标准：

第一部分：骨科护士Ⅰ类手术伤口管理现状调查与质性访谈研究 纳入标准：1.注册护士，非伤口专科护士；2.从事骨科临床护理工作满3年；3.具备独立完成问卷能力，愿意参与本研究。 第二部分：护士主导骨科Ⅰ类手术伤口管理循证实践方案的构建 纳入标准：1.注册护士；2.从事骨科临床护理工作＞3年；3.自愿参加本研究 第三部分：方案实施与效果评价（类实验研究） (一)患者样本（方案适用者） 纳入标准：1.年龄≥18岁；2.符合《外科手术部位感染预防与控制技术指南（试行）》中Ⅰ类手术伤口定义；3.无认知障碍并签署知情同意书。 (二）护士样本（方案使用者） 纳入标准：1.注册护士，非伤口专科护士；2.骨科临床工作≥3年；3.自愿参加研究。

Inclusion criteria

Part I: Investigation and Qualitative Study on the Current Status of Orthopedic Nurses in Class I Surgical Wound Management Inclusion Criteria:1.Registered nurses who are not wound-care specialists;2.Have at least three years of clinical experience in orthopedic nursing;3.Able to independently complete the questionnaire and willing to participate in the study. Part II: Development of an Evidence-Based Nurse-Led Orthopedic Class I Surgical Wound Management Program Inclusion criteria: 1. Registered nurse; 2. Engaged in orthopedic clinical nursing work for more than 3 years; 3. Voluntarily participate in this study. Part III: Implementation and Effect Evaluation (Quasi-Experimental Study) 1. Patient Sample (Program Recipients) Inclusion Criteria:1.Aged 18 years or older;2.Meet the definition of Class I surgical wound as described in the Guidelines for the Prevention and Control of Surgical Site Infection (Trial);3.Cognitively intact and able to provide written informed consent. 2. Nurse Sample (Program Users) Inclusion Criteria: 1. Registered nurses who are not wound care specialists; 2. At least 3 years of clinical experience in orthopedics; 3. Voluntary participation in the study.

第一部分：骨科护士Ⅰ类手术伤口管理现状调查与质性访谈研究。 （二）排除标准：1.正在外出进修或轮岗学习者；2.不在骨科一线从事护理工作的护士；3.研究期间离岗或无法完成问卷的人员。 第二部分：护士主导骨科Ⅰ类手术伤口管理循证实践方案的构建 （二）排除标准：1.进修护士；2.伤口专科护士 第三部分：方案实施与效果评价（类实验研究） （一）患者样本（方案适用者） 排除标准：1.合并慢性难愈性伤口（如压疮、糖尿病足等）；2.严重心、肝、肾功能障碍；3.恶性肿瘤患者；4.研究过程中退出或失访者。 (二）护士样本（方案使用者） 排除标准：1.进修护士；2.研究期间离岗或外出培训者。

Exclusion criteria：

Part I: Investigation and Qualitative Study on the Current Status of Orthopedic Nurses in Class I Surgical Wound Management Exclusion Criteria:1.Nurses currently participating in off-site training programs or rotational assignments;2.Nurses not engaged in frontline orthopedic clinical care;3.Individuals who leave their positions or are unable to complete the questionnaire during the study period. Part II: Development of an Evidence-Based Nurse-Led Orthopedic Class I Surgical Wound Management Program Exclusion Criteria:1. Continuing Education Nurse; 2. Wound Care Specialist Nurse Part III: Implementation and Effect Evaluation (Quasi-Experimental Study) 1.Patient Sample (Program Recipients) Exclusion Criteria:1.Patients with chronic non-healing wounds (e.g., pressure ulcers, diabetic foot ulcers);2.Patients with severe cardiac, hepatic, or renal dysfunction;3.Patients diagnosed with malignant tumors;4.individuals who withdraw or are lost to follow-up during the study period. 2.Nurse Sample (Program Implementers) Research Participants Exclusion Criteria:1.Nurses in advanced training or continuing education programs;2.Nurses who leave their positions or attend external training during the study period.
