研究目的：

通过前瞻性队列研究和人群综合干预研究，开展以社区为基础的心血管病主要危险因素的早期筛查、风险评估、综合干预和随访监测，（1）全面评估南京地区高危人群心血管病发病和死亡风险；（2）科学研发适合江苏省开展实施和推广运用的基于医疗物联网的社区心血管疾病综合干预适宜技术；（3）积极探索医疗物联网+慢性病防控，实施以社区卫生服务中心为主体，对社区心血管病高危人群和患者实施个性化、专业化和全程化、延续性卫生服务，构建医院、社区、家庭一体化的心血管病防控新模式；（4）长期建立前瞻性随访的重点人群专病队列和生物样本库，建立南京人群健康研究大数据；（5）大力构建临床医疗和公共卫生机构共同参与的心血管病防、治、管相结合“立体化防治”模式，合理配置医疗资源；（6）切实贯彻“健康中国2030”规划纲要，坚持以基层为重点，预防为主的工作方针，加强心血管病零级预防和一级预防。  

Objectives of Study：

Through prospective cohort and comprehensive population intervention studies, this project conducts community-based early screening, risk assessment, comprehensive intervention, and follow-up monitoring of major CVD risk factors. It aims to: (1) comprehensively evaluate CVD incidence and mortality risks in Nanjing’s high-risk groups; (2) scientifically develop community-based integrated CVD intervention technologies based on medical IoT suitable for implementation and promotion in Jiangsu; (3) explore medical IoT + chronic disease prevention, deliver personalized, professional, full-course, continuous health services for community CVD high-risk groups and patients via community health service centers, and establish an integrated hospital-community-family CVD prevention model; (4) long-term build a prospective disease-specific cohort of key populations, a biobank, and big data for Nanjing population health research; (5) construct a "three-dimensional prevention and treatment" model integrating CVD prevention, treatment, and management with joint participation of clinical and public health institutions, optimizing medical resource allocation; (6) implement the "Healthy China 2030" Planning Outline, adhere to primary care-focused and prevention-prioritized principles, and strengthen primordial and primary CVD prevention.

1.年龄在35-79岁； 2.社区常住居民（调查前12个月在项目区居住6个月以上的居民）； 3.纳入社区国家基本公共卫生服务管理。

Inclusion criteria

1. Aged 35 to 79 years; 2. Permanent residents of the community (residents who have lived in the project area for more than 6 months in the 12 months before the survey); 3. Included in the management of the national basic public health services of the community.

排除标准：

1.孕妇、存在认知障碍、严重疾病或残障。

1. Pregnant women, those with cognitive impairment, severe diseases or disabilities.
