研究目的：

（1）构建连续性术后管理体系： 针对ESD术后患者面临的随访脱节、基层承接能力不足、居家照护盲区等核心问题，整合医院、社区、家庭三方资源，设计职责明晰的三级联动路径：医院（主导个性化方案制定、危急处理及远程指导） 社区（承担日常随访、基础医疗干预及复诊协调）；家庭（落实居家护理与监测），建立覆盖"出院-社区-居家"全周期的无缝衔接管理机制。 （2）验证模式临床效能 通过随机对照试验，评估该模式对ESD术后患者并发症发生率、生活质量、自我管理能力及心理状态及医疗成本等指标的影响， 为模式推广提供循证医学证据。 （3）探索分级诊疗落地路径 依托互联网工具（微信平台、智能预警系统）实现三方数据实时共享，推动优质资源下沉，提升社区对复杂术后管理的承接能力，助力"康复回社区"政策实践。  

Objectives of Study：

(1) Establish a continuous post-operative management system: To address core issues faced by post-ESD patients, such as disjointed follow-up, insufficient capacity at the primary care level, and gaps in home care, this initiative integrates resources from hospitals, communities, and families to design a clearly defined three-tiered collaborative pathway: hospitals (leading the development of personalized plans, managing critical situations, and providing remote guidance), communities (responsible for routine follow-ups, basic medical interventions, and coordinating re-examinations), and families (implementing home care and monitoring). A seamless management mechanism covering the entire cycle from "discharge to community to home" will be established. (2) Validate the clinical efficacy of the model: Through a randomized controlled trial, the impact of this model on post-ESD patients will be evaluated, focusing on indicators such as complication rates, quality of life, self-management capabilities, psychological status, and healthcare costs. The aim is to provide evidence-based medical support for the broader implementation of the model. (3) Explore pathways for implementing tiered healthcare: Leveraging internet-based tools (e.g., WeChat platforms, intelligent early-warning systems) to enable real-time data sharing among the three tiers, this initiative promotes the dissemination of high-quality resources to the primary care level. It enhances the capacity of commu

纳入标准：

1.经病理确诊为消化道早癌且已行 ESD 治疗、年龄≥18岁、预期生存期≥ 12 个月、自愿参与本研究并签署知情同意书；

Inclusion criteria

1.Diagnosed with early gastrointestinal cancer through pathology and undergoing ESD treatment, aged >= 18 years, expected survival >= 12 months, voluntarily participating in this study and signing an informed consent form;

排除标准：

1.合并严重的心脑血管疾病、肝肾功能障碍、精神疾病或认知障碍等影响研究实施和结果判断的患者。

Exclusion criteria：

1.Patients with severe cardiovascular and cerebrovascular diseases, liver and kidney dysfunction, mental illness, or cognitive impairment that affect the implementation and outcome judgment of the study.
