研究目的：

通过近红外脑功能成像技术（fNIRS)研究冻结步态（FOG）患者在外部线索干预下的大脑皮层激活情况以及脑网络连接情况，从而探索外部线索干预改善FOG的深层机制。  

Objectives of Study：

The study aims to explore the deep mechanism by which external cueing intervention improves freezing of gait (FOG) through investigating the cortical activation and brain network connectivity of FOG patients under external cueing intervention using functional near-infrared spectroscopy (fNIRS).

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 根据2015年Movement Disorder Society（MDS）临床诊断标准诊断帕金森病（PD）； 2. 年龄50–80岁； 3. 根据步态冻结问卷（Freezing of Gait Questionnaire, FOG-Q）确认存在步态冻结； 4. 根据Hoehn-Yahr（H-Y）分期量表评定为II–III期PD； 5. 在无任何辅助设备的情况下具备独立行走能力； 6. 具有足够听力和视力（通过“耳语测试”正确回忆及使用Snellen图表达到6/12视力判断）； 7. 服用稳定剂量的药物至少4周。

Inclusion criteria

1. Diagnosis of Parkinson's disease (PD) according to the 2015 Movement Disorder Society (MDS) clinical diagnostic criteria; 2. Age between 50 and 80 years; 3. Presence of freezing of gait confirmed by the Freezing of Gait Questionnaire (FOG-Q); 4. PD staged as II–III according to the Hoehn-Yahr (H-Y) scale; 5. Ability to walk independently without any assistive devices; 6. Adequate hearing and vision (assessed by correct recall in the whisper test and visual acuity of 6/12 on the Snellen chart); 7. Stable medication dosage for at least 4 weeks.

排除标准：

1. 明显的认知障碍（蒙特利尔认知评估量表MoCA总分<26，或简易精神状态检查量表MMSE<24）； 2. 影响步态的其他疾病（如肌肉骨骼疾病、视力障碍、前庭功能障碍）； 3. 严重的体位性低血压； 4. 运动受限时间超过2分钟。

Exclusion criteria：

1. Significant cognitive impairment (Montreal Cognitive Assessment [MoCA] score < 26, or Mini-Mental State Examination [MMSE] score < 24); 2. Other conditions affecting gait (e.g., musculoskeletal disorders, visual impairment, vestibular dysfunction); 3. Severe orthostatic hypotension; 4. Exercise limitation lasting more than 2 minutes.

受试者需佩戴近红外脑功能成像设备（fNIRS）来监测大脑皮层激活情况以及脑网络连接情况，并且同时使用步态与姿势分析仪（RightGait&Posture）监测步幅，步速，步频等步态参数。我们将施加不同的外部线索干预措施，包括视觉线索引导，听觉线索引导以及视听结合线索引导。帕金森冻结步态人群需正常服用治疗帕金森的药物。

Intervention：

Participants are required to wear a functional near-infrared spectroscopy (fNIRS) device to monitor cortical activation and brain network connectivity, while simultaneously using a gait and posture analysis system (RightGait&Posture) to assess gait parameters such as step length, walking speed, and step frequency. Different external cueing interventions—including visual cueing, auditory cueing, and combined audiovisual cueing—will be applied. Participants with Parkinsonian freezing of gait must continue taking their prescribed Parkinson’s medication as usual.
