研究目的：

评估单孔机器人辅助胃肿瘤切除术的安全性及可行性，评估该手术的临床疗效。分析术中并发症、术后并发症、围手术期恢复效果及安全性、手术标本质量、病理学指标等，评估手术的安全性和可行性。  

Objectives of Study：

This study aims to evaluate the safety and feasibility of single-port robotic-assisted gastrectomy for gastric tumors, as well as to assess its clinical efficacy. The evaluation will focus on intraoperative complications, postoperative complications, perioperative recovery outcomes, specimen quality, and pathological parameters. These comprehensive metrics will collectively determine the procedural safety and feasibility.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄：18-75岁；
2.经内镜病理证实的胃腺癌；
3.根据第 7 版 AJCC 癌症分期手册术前评估为(内镜下无法切除的T1；或cT2-4a，N-/+，M0)；
4.可通过手术根治性切除；
5.ECOG评分 0-1分；
6.ASA分级 Ⅰ- Ⅲ级；
7.签署知情同意书的患者；

Inclusion criteria

1.Age 18–75 years.
2.Pathologically confirmed gastric adenocarcinoma by endoscopic biopsy.
3.Clinically staged as endoscopically unresectable T1, or cT2–4a, N–/+, M0 according to the AJCC Cancer Staging Manual, 7th edition.
4.Eligible for radical surgical resection.
5.ECOG performance status of 0 or 1.
6.ASA physical status class I–III.
7.Signed informed consent.

排除标准：

1.怀孕或哺乳期妇女； 2.严重精神障碍； 3.既往上腹部手术史； 4.既往胃切除术、ESD或EMR手术史； 5.术前影像学检查发现区域淋巴结肿大或直径超过3cm； 5年内有其他原发性肿瘤病史； 6.既往有新辅助化疗或放疗史； 7.既往6个月有心脑血管意外病史； 8.过去1个月有持续系统性使用皮质类固醇激素史； 9.需同时进行其他疾病手术； 10.因胃癌并发症（出血、梗阻或穿孔）需紧急手术； 11.既往参与过其他干预性临床试验； 12.严重传染病患者； 13.严重全身性疾病患者； 14.术前影像学检查发现区域淋巴结肿大或直径超过3cm。

Exclusion criteria：

1.Pregnancy or lactation.
2.Severe psychiatric disorder.
3.Previous upper abdominal surgery.
4.History of gastrectomy, endoscopic submucosal dissection (ESD), or endoscopic mucosal resection (EMR).
5.Preoperative imaging suggesting regional lymph node enlargement or diameter >3 cm.
6.History of other primary malignancies within 5 years, except appropriately treated non-melanoma skin cancer or carcinoma in situ.
7.Prior neoadjuvant chemotherapy or radiotherapy.
8.History of cerebrovascular accident or myocardial infarction within the past 6 months.
9.Systemic corticosteroid use for >1 month within the past 1 month.
10.Requirement for concurrent surgery for other diseases.
11.Emergency surgery due to complications of gastric cancer (e.g., bleeding, obstruction, or perforation).
12.Participation in another interventional clinical trial within the past 3 months.
13.Active severe infectious disease.
14.Severe systemic comorbidities deemed to contraindicate surgery.

本研究采用病例记录表（CRF）与电子数据采集系统（EDC）相结合的方式进行数据采集与管理。CRF经精心设计，涵盖人口学资料、术前评估、术中指标、术后病理及随访信息等全部研究变量，由研究者如实填写。所有数据通过EDC系统进行双人独立录入，系统内置逻辑校验功能，实时核查数据完整性与一致性，确保准确。EDC系统支持权限管理、数据溯源及电子签名，保障数据安全与合规。双轨制操作有效提升数据质量，为研究结论的可靠性奠定基础。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

In this study, case record form (CRF) combined with electronic data capture system (EDC) was used for data collection and management. The CRF was carefully designed to cover all study variables such as demographic data, preoperative evaluation, intraoperative indicators, postoperative pathological and follow-up information, and was filled in by the investigators. All data were independently entered by two people through the EDC system, and the system had built-in logic check function to verify data integrity and consistency in real time to ensure accuracy. The EDC system supports authority management, data traceability and electronic signature to ensure data security and compliance. The double-track operation can effectively improve the quality of data and lay the foundation for the reliability of the research conclusions.
