研究目的：

基于母婴微生物传递的关键作用及妊娠期糖尿病、自身免疫性疾病对母婴健康的潜在影响，本研究拟通过前瞻性纵向队列研究，系统解析不同疾病状态下母体菌群、母乳菌群及代谢谱的动态变化特征，并阐明其对新生儿微生物群成熟、免疫发育及器官功能的影响机制。具体目标包括： 1 揭示疾病特异性母体菌群特征； 2 解析母乳菌群与代谢物的传递路径； 3 评估新生儿菌群发育与健康结局； 4 探索GDM/自身免疫性疾病与母婴菌群互作机制； 5 筛选早期干预靶点。  

Objectives of Study：

Given the pivotal role of maternal-infant microbial transmission and the potential impact of gestational diabetes and autoimmune diseases on maternal-infant health, this study aims to conduct a prospective longitudinal cohort investigation to systematically analyze the dynamic changes in maternal microbiota, breast milk microbiota, and metabolic profiles across different disease states. It further seeks to elucidate the mechanisms by which these changes influence neonatal microbiota maturation, immune development, and organ function. Specific objectives include: 1. Reveal disease-specific maternal microbiome characteristics; 2. Characterize transmission pathways of breast milk microbiota and metabolites; 3. Evaluate neonatal microbiota development and health outcomes; 4. Explore the mechanisms of maternal-infant microbiota interactions in GDM/autoimmune diseases; 5. Identify early intervention targets.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

招募孕早期（8-12周）在上海国妇婴进行产前检查的孕妇，包括健康孕妇和确诊为APS或SS的孕妇，将APS或SS的孕妇入组为自身免疫性疾病组，在孕早期行HbA1c检测，以发现GDM的高风险人群。对所有孕妇在孕中期（24-28周）进行75g口服葡萄糖耐量试验（Oral Glucose Tolerance Test, OGTT），实验进一步将原健康孕妇分组为GDM组和对照组，纳入的孕妇在产妇年龄、BMI、生活习惯和病史方面进行匹配。

Inclusion criteria

Recruiting pregnant women undergoing prenatal checkups at Shanghai Maternity and Infant Hospital during early pregnancy (8-12 weeks), including healthy pregnant women and those diagnosed with APS or SS. Women with APS or SS will be enrolled in the autoimmune disease group. HbA1c testing will be performed during early pregnancy to identify high-risk populations for GDM.All pregnant women will undergo a 75g Oral Glucose Tolerance Test (OGTT) during mid-pregnancy (24-28 weeks). This test will further stratify the originally healthy pregnant women into a GDM group and a control group. The enrolled pregnant women will be matched for maternal age, BMI, lifestyle habits, and medical history.

排除已患有糖尿病、既往代谢性疾病、炎症性疾病、精神类疾病或其他家族性疾病和具有药物滥用史的孕妇。

Exclusion criteria：

Exclude pregnant women with pre-existing diabetes, prior metabolic disorders, inflammatory diseases, psychiatric conditions, other familial diseases, or a history of substance abuse.
