研究目的：

本研究以本院已存的 FFPE 结直肠癌样本为基础，通过多重免疫荧光（mIF）与数字病理学方法，系统比较 MSS 与 MSI 结直肠癌肿瘤微环境（TME）中免疫细胞组成、极化状态与空间分布的差异，重点聚焦 M2 型巨噬细胞极化的量化与空间学特征，进而评估其是否是导致 MSS 型结直肠癌对免疫检查点抑制剂（ICI）耐药的关键组织学因素，并探索其作为预后/疗效预测标志物与潜在干预靶点的可行性。  

Objectives of Study：

Based on the existing FFPE colorectal cancer samples in our hospital, this study systematically compared the differences in the composition, polarization state and spatial distribution of immune cells in the tumor microenvironment (TME) of MSS and MSI colorectal cancer through multiplex immunofluorescence (mIF) and digital pathology methods. The focus is on the quantitative and spatial characteristics of M2-type macrophage polarization, and then to evaluate whether it is a key histological factor leading to resistance to immune checkpoint inhibitors (ICI) in MSS-type colorectal cancer, and to explore its feasibility as a prognostic/therapeutic predictive marker and potential intervention target.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.病理组织学确诊为结直肠腺癌（原发灶）；
2.已在病理报告或分子检测中明确微卫星状态为 MSS 或 MSI（任一）并可查阅检测报告；
3.可用并保存良好的 FFPE 组织块或未染色切片；
4.对应患者临床资料可追溯（含年龄、性别、手术/取材日期、TNM 分期、是否接受 ICI 及随访结局）；
5.已取得用于科研的样本/数据使用许可；

Inclusion criteria

1.Colorectal adenocarcinoma (primary lesion) was confirmed by histopathology;
2.Have confirmed microsatellite status as MSS or MSI (either) in the pathology report or molecular test and can consult the test report;
3.Usable and well-preserved FFPE tissue blocks or unstained sections;
4.The clinical data of the corresponding patients were traceable (including age, gender, date of surgery/harvesting, TNM stage, whether receiving ICI and follow-up outcome);
5.Permission to use samples/data for scientific research has been obtained;

排除标准：

1.FFPE 标本因保存差、抗原大量降解、切片坏死/组织不足或技术原因导致无法进行 mIF 染色与分析；
2.标本组织学结构被严重破坏、无法代表原始肿瘤微环境；
3.标本来源不明、或合并其他原发肿瘤使判断混淆者；
4.临床资料严重缺失，无法完成主要分析或校正混杂因子者；

Exclusion criteria：

1.FFPE specimens could not be stained and analyzed for mIF due to poor preservation, massive antigen degradation, section necrosis/insufficient tissue or technical reasons;
2.The histological structure of the specimens was severely damaged and could not represent the original tumor microenvironment;
3.The origin of the specimen is unknown, or the judgment is confused by the combination of other primary tumors;
4.Clinical data were seriously missing to complete the primary analysis or to adjust for confounding factors;
