研究目的：

（1）了解泌尿造口患者延续护理服务需求，构建基于Triangle模型分层分级延续性护理干预方案。 （2）通过实证研究检验基于Triangle模型分层分级延续性护理干预方案的科学性、适用性和有效性。为提高泌尿造口患者的自我护理水平以及自我效能感、减少泌尿造口的并发症、提高生活质量提供新路径。  

Objectives of Study：

(1) Identify the continuity of care needs of urostomy patients and develop a tiered continuity of care intervention plan based on the Triangle model. (2) Validate the scientific rigor, applicability, and effectiveness of the tiered continuity of care intervention plan based on the Triangle model through evidence-based research. This approach offers a new pathway to enhance urostomy patients' self-care capabilities and self-efficacy, reduce urostomy complications, and improve quality of life.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合膀胱癌的诊断标准[4]，行回肠通道术或输尿管皮肤造口术患者； ②意识清楚，无言语交流沟通障碍； ③年龄≥18岁； ④患者知情同意，自愿参加本研究。

Inclusion criteria

1.Patients meeting diagnostic criteria for bladder cancer [4] who underwent ileal conduit or ureterostomy; 2. Patients with clear consciousness and no speech communication impairment; 3. Age >= 18 years; 4.Patients provided informed consent and voluntarily participated in this study.

排除标准：

①伴随严重心、脑、肾等脏器疾病； ②合并其它恶性肿瘤； ③出院后随访转至下级医院者。

Exclusion criteria：

1. Presence of severe cardiac, cerebral, renal, or other organ diseases; 2. Concurrent malignant tumors; 3. Patients transferred to lower-level hospitals for follow-up after discharge.
