研究目的:

主要目的:评估基层儿童早期发展干预项目对儿童发展水平提升的有效性。 次要目的: 评估基层儿童早期发展项目对儿童发育迟缓率下降、体格发育、血红蛋白水平、母乳喂养促进、辅食添加种类合格率、辅食添加频次合格率提升的有效性; 评估基层儿童早期发展项目对主要照养人养育知识提升、养育自我及动机提升、养育技能提升、养育行为改善、亲子互动水平提升、对主要照养人心理健康改善、家庭管教行为改善、家庭功能改善、综合家庭养育环境水平改善、儿童屏幕暴露水平降低以及对主要照养人生命质量提升、呼吸道感染疾病患病情况减少的有效性。  

Objectives of Study:

Main objective: To evaluate the effectiveness of the Early Childhood Development at the Primary Level Program Evaluation program in improving children's development level. Secondary objectives: Assess the effectiveness of the Early Childhood Development at the Primary Level Program Evaluation program in reducing the developmental delay rate, improving the physical development,, reducing the rate of anemia, improving the breastfeeding behavior, increasing the types of correct complementary feeding, increasing the frequency of correct complementary feeding, improving the level of parenting knowledge of primary caregivers, improving the level of parenting self-efficacy and motivation of primary caregivers, improving the level of parenting skills of primary caregiver, improving the level of parenting behavior of primary caregivers, improving the parent-child interactions between primary caregiver and child, improving the level of mental health of primary caregivers, reducing the family discipline behaviors, improving level of family function, improving the integrated family parenting environment, reducing the level of children's screen exposure, improving the primary caregiver's quality of life, reducing the respiratory infections in children.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.6-23月龄儿童;
2.儿童为足月健康婴儿(>=37孕周)不伴有明显疾病,单胎;
3.儿童有固定主要照养人,主要照养人年龄<=65岁,儿童及其主要照养人在12个月内没有离开本地居住的计划;
4.主要照养人愿意参加本项目,并且能够在规定时间前来随访;
5.优先招募居住在镇街卫生院2km范围内儿童,若数量不足则再扩展到4km范围内儿童;

Inclusion criteria

1.Children aged 6 to 23 months;
2. The child is a full-term healthy infant (>=37 weeks of gestation) without obvious diseases, and is a single fetus.
3. The child has a fixed primary caregiver whose age is no more than 65 years old, and neither the child nor the primary caregiver has any plan to leave the local area for residence within 12 months.
4. The main caregiver is willing to participate in this project and can come for follow-up at the prescribed time.
5. Prioritize the recruitment of children living within a 2-kilometer radius of town or sub-district health centers. If the number is insufficient, expand the recruitment to children within a 4-kilometer radius.

排除标准:

1.儿童出生体重小于2500g;
2.儿童患有影响生长发育的严重出生缺陷、遗传病或遗传代谢性疾病(如唐氏综合征、甲状腺功能低下、苯丙酮尿症等);
3.儿童新生儿期患有严重感染性疾病(如宫内感染、肺炎、败血症等)、高胆红素血症、新生儿惊厥、持续性低血糖等;
4.儿童存在产伤、宫内/产时/产后窒息、缺氧缺血性脑病或颅内出血,需住院治疗;
5.儿童已经由权威机构明确诊断为全面发育落后;
6.主要照养人诊断为抑郁症、精神分裂症等精神或神经系统疾;

Exclusion criteria:

1.Birth weight below 2500 grams;
2.Severe birth defects, genetic diseases or inborn errors of metabolism (such as Down syndrome, hypothyroidism, phenylketonuria, etc.);
3.Suffering from severe infectious diseases (such as intrauterine infection, pneumonia, sepsis, etc.), neonatal hyperbilirubinemia, neonatal convulsions, persistent hypoglycemia, etc. in the neonatal period;
4.Severe birth injury, such as intrauterine/intrapartum/postpartum asphyxia, hypoxic-ischemic encephalopathy or intracranial hemorrhage, and requiring hospitalization;
5.Diagnosed as Global Developmental Delay (GDD) by an authoritative agency;
6.The primary caregiver has been diagnosed with mental or neurological diseases such as depression and schizophrenia;

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

数据采集方案:门诊部分干预开展、养育照护小组开展、评估等均使用儿早项目数据采集系统进行数据采集;同时实施部分以及经费相关内容使用问卷、访谈等定性定量结合方法进行收集。 数据管理方案:本研究将采用电子数据采集(Electronic Data Capture, EDC)系统,研究数据将由授权的人员或受试者监护人直接录入到EDC系统中。数据录入前,对相关人员组织适当的数据采集培训,项目实施过程中需要接受质控人员的项目监查与数据质控。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data collection program: The outpatient part of the intervention, the parenting care group, and the evaluation will be collected using the Early Childhood Project Data Collection System; meanwhile, the implementation part and the cost-effectiveness content will be collected using a combination of qualitative and quantitative methods such as questionnaires and interviews. Data management plan: This study will use the Electronic Data Capture (EDC) system, and data will be entered directly into the EDC system by authorized personnel or guardians of the subjects. Before data entry, appropriate data capture training will be organized for relevant personnel, and the project will be subject to project monitoring and data quality control by QC personnel during project implementation.
