研究目的：

探讨握力、膈肌厚度、膈肌增厚分数、膈肌位移在预测ICU机械通气患者撤机结局中的价值，为将肌肉相关评价指标纳入撤机评估流程提供依据。  

Objectives of Study：

To investigate the value of grip strength, diaphragm thickness, diaphragm thickening fraction, and diaphragm excursion in predicting the weaning outcomes of mechanical ventilation in the ICU, so as to provide evidence for incorporating muscle-related evaluation indicators into the weaning assessment protocol.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄>=18岁； 2.气管插管或气管切开>=24小时； 3.通过SBT； 4.可以配合握力、超声检查； 5.受试者或授权委托人签署知情同意书。

Inclusion criteria

1. Age >= 18 years; 2. Endotracheal intubation or tracheostomy for >= 24 hours; 3. Successful passage of a spontaneous breathing trial (SBT); 4. Ability to cooperate with grip strength and ultrasound examinations; 5. Signed informed consent form obtained from the subject or their legally authorized representative.

排除标准：

1.妊娠期； 2.大量胸腔积液、气胸、膈下感染； 3.膈肌瘫痪； 4.因手术或外伤胸腹部无法取得超声切面； 5.气管插管>=21天仍未行气管切开患者； 6.决定撤除生命支持的患者。

Exclusion criteria：

1.Pregnancy; 2. Massive pleural effusion, pneumothorax, or subdiaphragmatic infection; 3. Diaphragmatic paralysis; 4. Inability to obtain adequate ultrasound views of the chest or abdomen due to surgery or trauma; 5. Endotracheal intubation retained for >=21 days without a tracheostomy; 6. Patients for whom a decision to withdraw life-sustaining therapy has been made.
