本研究旨在通过以下路径解决TKA术后功能评估与康复决策的临床与科学问题: 1)揭示关联机制: 量化步态参数(如膝关节六轴运动学、垂直地面反作用力峰值)与多维临床评分(SF-36、WOMAC、KSS)的非线性关联;识别影响术后生活质量的关键生物力学标志物(如步态对称性指数、屈曲活动度阈值)。 2)构建预测系统: 整合步态动力学数据、患者基线特征(BMI、术前影像学参数)及心理预期因素,开发基于机器学习的术后功能评分动态预测模型;实现早期预警(如预判KSS功能评分<70分)与个体化干预靶点推荐(如步态变异度异常患者的平衡训练强化)。  

Objectives of Study:

This study aims to address the clinical and scientific issues related to functional assessment and rehabilitation decision-making after Total Knee Arthroplasty (TKA) through the following approaches: Revealing the association mechanisms:Quantify the non-linear associations between gait parameters (e.g., knee joint six-axis kinematics, peak vertical ground reaction force) and multi-dimensional clinical scores (SF-36, WOMAC, KSS); identify key biomechanical biomarkers that affect post-operative quality of life (e.g., gait symmetry index, flexion range of motion threshold). Constructing a prediction system:Integrate gait kinetic data, patients' baseline characteristics (BMI, pre-operative imaging parameters) and psychological expectation factors to develop a machine learning-based dynamic prediction model for post-operative functional scores; enable early warning (e.g., predicting a KSS functional score < 70) and recommendation of individualized intervention targets (e.g., enhanced balance training for patients with abnormal gait variability).

纳入标准:

1)年龄≥18岁,接受初次单侧全膝关节置换术(TKA); 2)术后随访时间≥6个月,且保存完整步态分析数据(Opti_Knee系统)及临床评分(SF-36、WOMAC、KSS); 3)术前诊断为原发性骨关节炎。

Inclusion criteria

1.Aged >= 18 years, who underwent primary unilateral total knee arthroplasty (TKA); 2.Postoperative follow-up duration ≥ 6 months, with complete preservation of gait analysis data (Opti_Knee system) and clinical scores (SF-36, WOMAC, KSS); 3.Preoperative diagnosis of primary osteoarthritis.

排除标准:

1)合并其他下肢关节疾病(如髋/踝关节炎)或脊柱严重退变; 2) 存在神经肌肉疾病(如脑卒中后遗症、帕金森病); 3)术后发生假体感染、松动或翻修手术; 4)数据缺失率>20%(如步态参数或评分记录不全)。

Exclusion criteria:

1 Comorbidity with other lower extremity joint diseases (e.g., hip/ankle arthritis) or severe spinal degeneration; 2 Presence of neuromuscular disorders (e.g., post-stroke sequelae, Parkinson's disease); 3 Postoperative occurrence of prosthetic infection, loosening, or revision surgery; 4 Data missing rate > 20% (e.g., incomplete recording of gait parameters or scores).
