研究目的：

1.主要目的：探索运动与牙周健康之间的关系，明确运动在牙周炎防控中的潜在保护作用及其机制。 2.次要目的：初步鉴定运动后血浆和唾液中潜在的有益于牙周健康的代谢产物或生物因子；探索通过外源性补充相关代谢物间接模拟运动干预对牙周组织的有益影响，为后续机制研究和干预策略开发奠定基础。  

Objectives of Study：

Primary Objective: To investigate the relationship between physical activity and periodontal health, and to clarify the potential protective role of exercise in the prevention and management of periodontitis, as well as the underlying mechanisms. Secondary Objective: To preliminarily identify metabolites or biological factors in plasma and saliva that may benefit periodontal health following exercise, and to explore whether exogenous supplementation of these metabolites can indirectly mimic the beneficial effects of exercise on periodontal tissues, providing a foundation for subsequent mechanistic studies and intervention strategy development.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）诊断为牙周病（牙龈炎或牙周炎）； （2）年龄为18-35岁，性别不限； （3）一般健康状况良好，无明确诊断的全身系统性疾病（如糖尿病、心血管疾病、自身免疫疾病等）； （4）自愿参与研究，并签署知情同意书； （5）属于久坐不运动人群，具体包括：日常以静坐、躺卧等低能量消耗（<1.5 METs）活动为主，包括办公、驾驶、看电视等，符合世界卫生组织对“久坐行为”的定义；每日静坐/久坐总时长 ≥8 小时；

Inclusion criteria

1.Diagnosed with periodontal disease (gingivitis or periodontitis). 2.Aged 18–35 years, any gender. 3.Generally healthy, without a confirmed diagnosis of systemic diseases such as diabetes, cardiovascular diseases, or autoimmune disorders. 4.Voluntarily willing to participate in the study and able to provide written informed consent. 5.Sedentary lifestyle, defined as predominantly low-energy activities (<1.5 METs) such as sitting, lying down, office work, driving, or watching TV, in line with the World Health Organization definition of sedentary behavior, with total daily sitting time >= 8 hours.

排除标准：

（1）既往或当前患有严重系统性疾病（如糖尿病、冠心病、慢性肝肾疾病、神经系统疾病等）者； （2）存在急性牙周感染或近期（1个月内）接受过牙周治疗、抗生素治疗者； （3）处于妊娠或哺乳期者； （4）有吸烟史或正在吸烟者（因其对牙周健康和运动代谢具有显著干扰作用）； （5）有药物或酒精滥用史，或心理/行为障碍影响依从性者； （6）无法完成运动干预或体力活动受限者（如存在骨骼肌肉系统疾病、严重肥胖、运动损伤史等）； （7）不愿签署知情同意书者或依从性差者。

Exclusion criteria：

1.History or current presence of severe systemic diseases, such as diabetes, coronary heart disease, chronic liver or kidney disease, or neurological disorders. 2.Presence of acute periodontal infection or receipt of periodontal treatment or antibiotic therapy within the past month. 3.Pregnancy or breastfeeding. 4.Current smokers or those with a history of smoking, due to its significant impact on periodontal health and exercise metabolism. 5.History of drug or alcohol abuse, or psychological/behavioral disorders affecting adherence. 6.Inability to complete the exercise intervention or limitations in physical activity, such as musculoskeletal disorders, severe obesity, or history of exercise-related injuries. 7.Unwillingness to sign the informed consent form or poor adherence to study procedures.

This study employs randomized allocation to minimize the risk of selection bias. The random number sequence is generated by an independent biostatistician, not involved in the study team, using a computer-based random number generator (e.g., R or SAS), with a randomization ratio of 2:1 (exercise group: control group). The generated random sequence includes only the participant ID and corresponding group code, and is securely kept by the independent biostatistician, who does not participate in participant recruitment or intervention implementation.

盲法：

本研究采用盲法的主要目的是降低实施偏倚和测量偏倚，确保临床检测和数据分析的客观性。 1.受试者盲法：由于运动干预的性质，受试者在干预过程中 知道自己是否参加运动组（开放标签），因此无法对受试者进行盲法。 2.临床评估人员盲法：所有临床检测人员在收集牙周健康指标（菌斑指数、出血指数、牙龈指数）、唾液炎症因子、口腔菌斑及血液代谢组样本时 不知受试者的分组情况。检测人员仅根据受试者编号进行样本采集和临床检查，组别信息由独立数据管理员保管，直到数据分析完成后才可解盲。 3.数据分析人员盲法：对于实验数据的录入、整理和统计分析，数据分析人员也 不知受试者的组别，仅使用编码数据（受试者编号）。 4.分析完成并确定统计结果后，由独立管理员揭示分组信息进行最终解释。

Blinding：

Participant blinding: Due to the nature of the exercise intervention, participants are aware of whether they are in the exercise group during the intervention (open-label), making participant blinding infeasible. Clinical assessor blinding: All clinical examiners remain unaware of participants’ group assignments when collecting periodontal health indicators (plaque index, bleeding index, gingival index), saliva inflammatory markers, oral plaque, and blood metabolomics samples. Assessors perform sample collection and clinical examinations based solely on participant IDs, while group assignment information is securely maintained by an independent data manager and only revealed after data analysis is completed. Data analyst blinding: Data analysts are blinded to group assignments during data entry, organization, and statistical analysis, working exclusively with coded data (participant IDs). Unblinding: After the analysis is completed and statistical results are finalized, the independent data manager will reveal the group assignments for final interpretation.
