研究目的：

观察经颅交流电刺激改善脑卒中后上肢运动功能的即时效应。  

Objectives of Study：

To observe the immediate effects of transcranial alternating current stimulation on upper limb motor function after stroke.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.经CT/MRI确诊为缺血性/出血性脑卒中； 2.年龄 >=45周岁且 <=80周岁，性别不限； 3.小学及以上文化程度； 4.初次发病，病程 >=3月，单侧偏瘫； 5.无明显认知障碍，简易精神状态检查（Mini-Mental State Examination, MMSE）：小学文化 > 20分；中学及以上文化 > 24分； 6.单侧上肢运动功能障碍，Brunnstrom分期 >= II期； 7.自愿参加并签署知情书。

Inclusion criteria

1. Diagnosed ischemic/hemorrhagic stroke by CT/MRI; 2. Age >=45 years old and <=80 years old, gender is not limited; 3. Primary school education or above; 4. Initial onset, course of >=3 months, unilateral hemiplegia; 5. No obvious cognitive impairment, Mini-Mental State Examination (MMSE): Primary school culture > 20 points; Secondary school education and above> 24 points; 6. Unilateral upper limb motor dysfunction, Brunnstrom stage > = stage II; 7. Participate voluntarily and sign the form of inform.

排除标准：

1.急性期生命体征不稳定者； 2.失语症、听力障碍等无法与他人正常言语沟通者； 3.严重骨关节及肌肉疾病者； 4.妊娠期或哺乳期妇女； 5.严重脏器疾病及严重精神疾病者，包括但不限于以下疾病：心脏疾病（如心肌梗死、心房颤动）、肝脏疾病（如肝功能衰竭、肝癌）、肾脏疾病（如终末期肾病）肺部疾病（如呼吸衰竭）、精神疾病（如精神分裂症）； 6.存在经颅交流电刺激（transcranial alternating current stimulation, tACS）和脑电图（EEG）禁忌证，如心脏起搏器、心脏支架、幽闭恐惧症、颅脑外伤或颅骨缺损，头皮有感染、伤口等； 7.癫痫病史患者； 8.入组前一个月参加过或者正在参加其他的临床课题项目。

Exclusion criteria：

1. Those with unstable vital signs in the acute phase; 2. Those who are unable to communicate with others normally such as aphasia and hearing impairment; 3. Patients with severe bone, joint and muscle diseases; 4. Pregnant or lactating women; 5. Severe organ diseases and severe mental diseases, including but not limited to the following diseases: heart diseases (such as myocardial infarction, atrial fibrillation), liver diseases (such as liver failure, liver cancer), kidney diseases (such as end-stage renal disease), lung diseases (such as respiratory failure), mental diseases (such as schizophrenia); 6. Contraindications to transcranial alternating current stimulation (tACS) and electroencephalography (EEG), such as pacemaker, cardiac stent, claustrophobia, head trauma or skull defect, scalp infection, wounds, etc.; 7. Patients with a history of epilepsy; 8. Participated in or are participating in other clinical projects one month before enrollment.

经颅交流电刺激+院内基础治疗: 经颅交流电刺激：受试者会被要求坐在舒适的躺椅上，关闭智能手机，并被鼓励喝水、放松。三个Nexalin公司生产的电极（NexalinTechnology, Inc., Houston, TX, USA; https://nexalin.com）应用于头皮上：一个电极放置在前额，电极大小为4.45×9.53cm，两个电极分别放置在左右乳突区域，大小为3.18×3.18cm。输出的电流大小为15mA，频率为77.5Hz，治疗时长为40分钟，共1次。

Transcranial alternating current stimulation In-hospital basic treatment: Transcranial alternating current stimulation: Subjects are asked to sit in a comfortable recliner, turn off their smartphones, and are encouraged to drink water and relax. Three electrodes manufactured by Nexalin (Nexalin Technology, Inc., Houston, TX, USA; https://nexalin.com) Apply to the scalp: one electrode is placed on the forehead, the size of the electrode is 4.45×9.53cm, and the two electrodes are placed in the left and right mastoid areas respectively, the size is 3.18×3.18cm. The output current is 15 mA, the frequency is 77.5 Hz, and the treatment duration is 40 minutes, a total of 1 time.
