研究目的：

退行性腰椎失稳是临床中引发腰痛的常见疾病，该病迁延难愈、反复发作，严重影响患者生活质量。根据相关调查显示，腰痛常年位居发达国家和欠发达国家疾病负担指标的首位。在我国腰痛导致的疾病负担同样位居第一，目前国内外对于该病的研究投入也逐年增加，我们希望通过坐位腰椎旋转手法及仰卧腰椎牵引法来治疗本病，一方面为病人减轻病患的痛苦，另一方面科学分析坐位腰椎旋转手法治疗该病并进行评价。  

Objectives of Study：

Degenerative lumbar instability is a common disease that causes low back pain in clinic. The disease can not be cured and recurred, which seriously affects the quality of life of patients. According to the relevant survey, low back pain ranks first in the disease burden indicators of developed and underdeveloped countries all the year round. In our country, the disease burden caused by low back pain also ranks first. At present, the research investment in this disease is increasing year by year at home and abroad. We hope to treat this disease by seated lumbar rotation manipulation and Lumbar traction in supine positionn, on the one hand, to alleviate the patients' pain, on the other hand, to scientifically analyze and evaluate the seated lumbar rotation manipulation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 符合退行性腰椎失稳的诊断标准：
a.明显、反复的腰痛，及有严重的酸痛或无力感；
b.局限性腰痛和/或伴下肢牵涉痛；
c.MRI或CT等检查可发现有明显腰椎间盘、椎间关节等退变并排除其它疾病；
d.X线片示椎体前缘有牵拉骨赘形成或椎间隙明显狭窄, 在腰椎动力位片 (过伸、过屈侧位X线平片) 病变相邻2个椎体间滑移>3mm, 病椎不超过两个节段, 病变2个椎体相对成角>11°。
2. 年龄为45~65岁，性别不限。
3．自愿参加实验，并签署知情同意书。

Inclusion criteria

1. Meet the diagnostic criteria of degenerative lumbar instability:
(1) Obvious and repeated low back pain, and severe pain or weakness;
(2) Local low back pain and / or lower extremity involvement pain;
(3) MRI or CT can find obvious degeneration of lumbar intervertebral disc and joint and exclude other diseases;
(4) X-ray film showed that there was distraction osteophyte formation or obvious stenosis of intervertebral space in the front of the vertebral body. In the dynamic position film (hyperextension and hyperflexion lateral X-ray film) of the lumbar spine, the slip between the two adjacent vertebral bodies was more than 3mm, the disease vertebral body was not more than two segments, and the relative angle of the two vertebral bodies was more than 11 °.
2. Aged 45-65 years old;
3. Volunteer to participate in the experiment and sign the informed consent.

排除标准：

1. 先天性脊柱病变造成的失稳。
2. 脊柱感染、创伤骨折、肿瘤、结核、骨质疏松造成的失稳。
3. 合并严重心、脑血管、肝肾和造血系统、内分泌系统疾病。
4. 精神疾病或老年痴呆患者。
5. 发育不良型、峡部裂型、创伤型、病理型的腰椎失稳。
6. 手法部位有严重皮肤损伤或皮肤病者。

Exclusion criteria：

1. Instability caused by congenital spinal diseases.

2. Instability caused by spinal infection, traumatic fracture, tumor, tuberculosis and osteoporosis.

3. Serious heart, cerebrovascular, hepatorenal, hematopoietic and endocrine diseases.

4. Patients with mental illness or Alzheimer's disease.

5. Lumbar instability of dysplasia, isthmus, trauma and pathology.

6. Severe skin injury or skin disease at the manipulation site.

采用中央随机系统将60例符合本研究入选标准的退行性腰椎失稳患者随机分为试验组与对照组，两组各30例。试验组予以坐位腰椎旋转手法治疗，对照组予以仰卧位腰椎牵引治疗。
