本研究旨在探讨基于分级分型的个性化运动处方对AIS患者脊柱结构、核心肌群功能、姿态控制及生活质量的改善效果,并比较其与常规运动训练的差异。同时,拟通过亚组分析明确不同分级、分型患者对运动干预的响应特征,从而为AIS青少年提供循证支持的个性化运动干预方案。  

Objectives of Study:

This study aims to investigate the effects of personalized exercise prescriptions based on classification and grading on spinal structure, core muscle function, postural control, and quality of life in patients with Adolescent Idiopathic Scoliosis (AIS). Additionally, it seeks to compare these effects with those of conventional exercise training. Subgroup analyses will be conducted to identify the response characteristics of patients with different classifications and gradings, in order to provide evidence-based personalized exercise interventions for adolescents with AIS.

纳入标准:

(1)年龄 10~18 岁,X 线确诊为AIS; (2)Cobb 角在 10°~ 40°之间; (3)未接受过脊柱手术,且近期无系统性康复治疗史; (4)能够配合运动训练,智力水平正常,能理解任务要求; (5)自愿参加,监护人签署知情同意书。

Inclusion criteria

(1) Aged between 10 and 18 years, diagnosed with Adolescent Idiopathic Scoliosis (AIS) by X-ray imaging; (2) Cobb angle between 10° and 40°; (3) No history of spinal surgery and no recent history of systematic rehabilitation treatment; (4) Able to cooperate with exercise training, with normal cognitive function and the ability to understand task instructions; (5) Participation is voluntary, and informed consent has been signed by the legal guardian.

排除标准:

(1)继发性脊柱侧弯(如神经肌肉性、先天性侧弯); (2)合并严重心肺疾病、神经系统疾病或运动禁忌症; (3)既往脊柱手术史或需紧急手术者; (4)近期使用影响运动功能的药物或存在严重疼痛影响训练者; (5)不具备规律随访条件或依从性差者。

Exclusion criteria:

(1) Secondary scoliosis (e.g., neuromuscular or congenital scoliosis); (2) Presence of severe cardiopulmonary diseases, neurological disorders, or contraindications to exercise; (3) History of spinal surgery or requiring urgent surgical intervention; (4) Recent use of medications affecting motor function or experiencing severe pain that interferes with training; (5) Inability to comply with regular follow-up or poor adherence to the intervention protocol.

(1)训练形式方面,内容主要是根据患者分型实施个性化的特定矫正运动、姿势稳定训练和呼吸训练,具体内容包括依据患者分型个性化的特定矫正运动、姿势稳定训练和旋转成角呼吸训练,对患者进行脊柱三维方向的畸形矫正,训练顺序为骨盆位置矫正、仰卧位、侧卧位、俯卧位三维矫正。(2)训练强度方面,运动强度的调节通过增加或减少持续时间、动作进阶和退阶、扩大或减小关节活动范围、加快或减慢运动速度实现。(3)训练频率与时间方面,干预2次/周,60min/次,共12周。(4)具体实施分为三个部分:准备活动:5min的热身运动,主要是活动四肢和躯干关节、拉伸大肌肉群;分级分型运动处方干预方案实施:50 min的运动康复干预训练,具体内容因每个患者的分级分型而各不相同,主要内容为个性化的特定矫正运动、姿势稳定训练和呼吸训练。整理活动:5min的放松活动,主要是小强度的、四肢肌肉牵伸活动,以放松紧张的肌肉群。

Intervention:

(1) Training Content:The program consists of personalized corrective exercises, postural stability training, and breathing exercises, tailored according to the patient's scoliosis classification. Specifically, individualized corrective exercises, posture stabilization training, and rotational angular breathing (RAB) techniques are applied to address spinal deformities in three-dimensional planes. The training sequence includes pelvic alignment correction, followed by three-dimensional spinal correction in supine, side-lying, and prone positions.(2) Training Intensity:Exercise intensity is adjusted by modifying factors such as duration, movement progression or regression, range of joint motion, and movement speed.(3) Training Frequency and Duration:Intervention is administered twice per week, 60 minutes per session, over a total period of 12 weeks.(4) Training Structure: The intervention session is divided into three phases:Warm-up (5 minutes): Includes general mobility exercises for limbs and trunk joints, as well as dynamic stretching of major muscle groups. Main Intervention (50 minutes): Implementation of the graded and classified exercise prescription, with personalized training content based on each patient’s classification and grading. The main components include specific corrective exercises, postural stability training, and breathing exercises.Cool-down (5 minutes): Comprises low-intensity stretching exercises targeting limb muscles, aiming to relax tense muscle groups.
