本研究旨在探讨癫痫患者在听觉语言任务执行过程中,皮层不同脑区的神经响应特征及其功能连接模式,并借助SEEG技术对语言加工相关脑网络进行精准定位。 本研究将以接受SEEG电极植入的癫痫患者为研究对象,采用语音或语义听觉刺激范式,在其麻醉状态下及清醒状态下记录SEEG数据。通过对不同刺激条件下的SEEG信号进行时间频率分析、事件相关电位提取和脑区间功能连接建模,识别与语音感知、语义理解相关的大脑皮层关键区域及其时空动态特征。 本研究的核心假设为:癫痫患者在执行语言听觉任务过程中,特定脑区(如颞上回、岛叶、额下回等)在特定时间窗及频段内表现出显著的神经活动变化,且不同任务条件下脑区间的功能连接模式存在差异。 预期研究结果包括:明确与语言加工密切相关的SEEG电极位点及其对应皮层区域;描述不同任务条件下皮层响应的时间-频率特征;构建语言任务相关的功能连接网络,为癫痫术前语言区的电生理定位提供依据。 本研究最终目标是为癫痫术前功能区保护提供数据支持,推动脑功能区“可视化”图谱的临床转化。 |

Objectives of Study: |
This study aims to explore the neural response characteristics and functional connection patterns of different brain regions in epileptic patients during the execution of auditory language tasks, and to precisely locate the language processing-related brain networks using SEEG technology. This study will focus on epileptic patients who have undergone SEEG electrode implantation. It will employ a speech or semantic auditory stimulation paradigm and record SEEG data under anesthesia and in a waking state. Through time-frequency analysis of SEEG signals, extraction of event-related potentials, and modeling of functional connections between brain regions, the key brain cortical areas related to speech perception and semantic understanding and their spatiotemporal dynamic characteristics will be identified. The core hypothesis of this study is that during the execution of language auditory tasks, specific brain regions (such as the superior temporal gyrus, insula, and inferior frontal gyrus, etc.) exhibit significant neural activity changes within specific time windows and frequency bands, and the functional connection patterns between brain regions vary under different task conditions. The expected research results include: identifying the SEEG electrode sites closely related to language processing and their corresponding cortical regions; describing the time-frequency characteristics of cortical responses under different task conditions; constructing a functional connection network related to language tasks, providing a basis for preoperative electrophysiological localization of language areas in epilepsy surgery. The ultimate goal of this study is to provide data support for preoperative functional area protection in epilepsy and promote the clinical translation of the "visualization" map of brain functional areas. |

纳入标准: |
1.2018 年-2026 年间在首都医科大学宣武医院功能神经外科住院或首都医科大学附属北京天坛医院治疗的癫痫患者的临床数据; 2.临床数据采集时的年龄限制为 4 岁以上,60 岁以下(参考 FDA 对迷走神经电刺激(VNS)的年龄限制为最小 4 岁以上,2013 年 FDA 对反应性神经电刺激 RNS 的年龄限制为 18 岁,考虑到未来可能放宽最小年龄限制,需要使预测模型适应儿童的脑电模式); 3.癫痫类型属于以下几类: (1)发作起源涉及颞叶的; (2)发作起源涉及运动皮层的; (3)发作起源涉及顶枕叶的; (4)肿瘤或其他局灶性病理改变继发的癫痫; 4.在住院期间进行了常规的癫痫术前评估,并进行的必要的影像学、脑电图学检查的患者,且检查原始数据可获取的。具体而言:影像学检查包括头部磁共振扫描的结构相(三维磁化准备快速梯度回波序列 3D MP-RAGE)、电极埋置术后薄扫 CT(用于定位电极位置);其他影像学检查包括脑磁图(Magnetoencephalography, MEG)、正电子发射型计算机断层显像(Positron Emission Computed Tomography,PET)在本研究中作为患者分组的依据;脑电图学检查包括头皮脑电图(EEG)、术中硬膜下电极监测脑电图记录(ECoG)以及立体定向脑电图(SEEG) 。 |

Inclusion criteria |
1. Clinical data of epilepsy patients treated in the Functional Neurosurgery Department of Xuanwu Hospital, Capital Medical University, or Beijing Tiantan Hospital affiliated with Capital Medical University from 2018 to 2026; 2. The age limit for data collection is set for those aged 4 and above, and under 60 (based on the FDA's age limit for Vagus Nerve Stimulation (VNS) being at least 4 years old, and 18 years for Responsive Neurostimulation (RNS) established in 2013; considering that the minimum age limit may be relaxed in the future, it is necessary to adapt the predictive model to the EEG patterns of children); 3. The types of epilepsy fall into the following categories: (1) Seizures originating in the temporal lobe; (2) Seizures originating in the motor cortex; (3) Seizures originating in the parietal-occipital lobe; (4) Epilepsy secondary to tumors or other focal pathological changes; 4. Patients who underwent routine pre-surgical evaluations for epilepsy during hospitalization, and underwent necessary imaging and electroencephalographic examinations with accessible raw data. Specifically, imaging examinations included structural imaging of the head using MRI (three-dimensional magnetization prepared rapid gradient echo sequence 3D MP-RAGE), and post-electrode implantation thin-cut CT (for locating electrode positions); other imaging examinations included Magnetoencephalography (MEG) and Positron Emission Tomography (PET) which served as the basis for patient grouping in this study; electroencephalographic examinations included scalp electroencephalography (EEG), intraoperative subdural electrode monitoring EEG recording (ECoG), and stereotactic electroencephalography (SEEG). |

1、患者脑电图数据保存不完整,保存的发作期脑电图数据少于三次的; 2、患者因各种原因终止了外科治疗的; 3、其他研究者认为不适合参与本研究的。 |

Exclusion criteria: |
1. The patient's EEG data was not fully preserved, and the number of recorded seizure EEG data was less than three times. 2. The patient terminated the surgical treatment for various reasons. 3. Other researchers considered themselves ineligible to participate in this study. |
