研究目的：

主要目的：本研究拟建立一套直肠癌影像-病理结节一一对应方法，以病理诊断为金标准，前瞻性验证MRI诊断肿瘤沉积物（TDs）的准确性。 次要目的： 1. 分析TDs的MRI的影像特征，进而构建基于影像学特征的 TDs 诊断标准。 2. 明确影像学及病理诊断的TDs对局部复发、无病生存期和总体生存期的影响。 3. 评估病理学家之间以及放射科医生之间的观察者间一致性。 4. 基于病理金标准分析、验证MRI图像中鉴别 TDs 与 EMVI、淋巴结转移的方法。  

Objectives of Study：

Main Objective: This study aims to establish a one-to-one correspondence method between rectal cancer imaging and pathological nodules, using pathological diagnosis as the gold standard, and prospectively verify the accuracy of MRI in diagnosing tumor deposits (TDs). Secondary Objectives: 1. Analyze the MRI imaging features of TDs and construct a diagnostic standard for TDs based on imaging features. 2. Clarify the impact of imaging and pathological diagnosis of TDs on local recurrence, disease-free survival, and overall survival. 3. Evaluate the interobserver agreement between pathologists and radiologists. 4. Analyze and validate methods for distinguishing TDs from extramural vascular invasion (EMVI) and lymph node metastasis in MRI images based on the pathological gold standard.

纳入标准：

1. 年龄范围：患者年龄>=18 岁； 2. 经病理组织学诊断确诊为原发性直肠腺癌的患者； 3. 具有完整的盆腔MRI资料； 4. 接直肠癌切除手术的患者（包括术前接受治疗的患者）； 5. 患者自愿参与本研究，并签署书面知情同意书，愿意配合随访。

Inclusion criteria

1. Age range: Patients must be >= 18 years old; 2. Diagnosed with primary rectal adenocarcinoma by pathological histology; 3. Possess complete pelvic MRI data; 4. Patients who have undergone rectal cancer resection surgery (including those who received treatment before surgery); 5. Patients volunteer to participate in this study, sign a written informed consent form, and agree to cooperate with follow-up.

排除标准：

1. 患者就诊时患有复发性肿瘤、同步性肿瘤、家族性腺瘤性息肉病或炎症性肠病； 2. 无法进行MRI或影像学资料图像质量差； 3. 具有任何可能影响方案依从性的疾病史（如严重精神障碍、认知功能障碍、药物滥用或成瘾等）； 4. 妊娠或哺乳期妇女，或有生育能力且不愿/无法采取有效避孕措施者； 5. 检查前 6 个月内曾参加过任何药物临床试验者。

Exclusion criteria：

1. The patient was diagnosed with recurrent tumors, synchrony tumors, familial adenomatous polyposis or inflammatory bowel disease at the time of the visit. 2. MRI cannot be performed or the image quality of imaging data is poor; 3. Have any medical history that may affect compliance with the protocol (such as severe mental disorders, cognitive dysfunction, drug abuse or addiction, etc.); 4. Pregnant or lactating women, or those who are fertile but unwilling or unable to take effective contraceptive measures; 5. Those who have participated in any drug clinical trials within the six months prior to the examination.

组织病理学评估，即病理诊断：由2名病理学家通过光学显微镜对苏木精和伊红染色的组织载玻片评估CT所识别的结节是否为肿瘤沉积。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

The histopathological evaluation: two pathologists assess the nodules identified by CT through the use of an optical microscope on tissue slides stained with hematoxylin and eosin to determine if they are tumor deposits.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Patient details, including demographic and clinical characteristics such as age at diagnosis, gender, tumor location (High, middle and low rectum), as well as preoperative carcinoembryonic antigen and CA19-9 levels, were retrieved and collected through the hospital's medical record system. The data will be anonymized by using the unique patient identification number (ID) assigned within the hospital system. Radiological and pathological results as well as treatment data will also be recorded.
