研究目的:

1. 主要目的: (1)通过循证药学结合群体药代动力学研究,明确霉酚酸类药物群体药代动力学关键影响因素; (2)基于医院电子病历数据,采用机器学习算法,构建并初步验证霉酚酸类药物剂量精准预测模型,为霉酚酸类药物精准治疗提供技术支撑。 2. 次要目的: 进一步提高医院精准用药水平,进一步保证药物治疗的安全性和有效性。  

Objectives of Study:

1. Main purpose: (1) Through the combination of evidence-based pharmacy and population pharmacokinetics study, to identify the key population pharmacokinetics factors of mycophenolic acids; (2) Based on the hospital electronic medical record data, the machine learning algorithm was used to build and preliminarily verify the accurate prediction model of mycophenolic acid drug dose, providing technical support for the precise treatment of mycophenolic acid. 2. Secondary purpose: Further improve the level of precision medication in hospitals, and further ensure the safety and effectiveness of drug treatment.

纳入标准:

1.使用霉酚酸类药物的住院患者;
2.年龄18~70岁;
3.以固定剂量服用霉酚酸类药物大于5天(即服药达稳态);
4.具有霉酚酸类药物治疗药物监测数据及临床资料的患者;

Inclusion criteria

1.Hospitalized patients using mycophenolic acids;
2.The age ranged from 18 to 70 years;
3.Taking mycophenolic acids at a fixed dose for more than 5 days (i.e. reaching a steady state);
4.Patients with mycophenolate treatment drug monitoring data and clinical data;

排除标准:

1.未规律服用霉酚酸类药物者;
2.住院期间未监测霉酚酸类药物患者;
3.孕妇及哺乳期妇女;
4.合并肿瘤者;

Exclusion criteria:

1.Those who do not take mycophenolate drugs regularly;
2.Patients with mycophenolate drugs not monitored during hospitalization;
3.Pregnant and lactating women;
4.Those with tumor.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

(1)数据汇编: 实时数据汇总,及时地编辑浓度-时间 数据、药物给药史和协变量数据。研究方案中记录判断数据可用或不可用的标准(比如缺少采集血样的时间、没有相关浓度信息的给药资料、缺少给药资料的浓度信息)。建立标准操作规程规范管理数据(包括数据的输入、存储、验证、校正和检索)和进行统计处理的方法和软件的质量与有效性。 (2)缺失数据处理: 本研究收集到的数据不能直接用于模型开发,需要进行数据的预处理。首先将收集好的数据录入Excel表格中进行保存。在数据预处理中,本研究首先对所获得的数据进行缺失值分析处理、文本数据结构化处理等预处理,其过程如下: ①对离群异常值和缺失值分布情况进行检查; ②对于缺失或异常的数据,根据不同变量的缺失情况和重要程度,分别采取删除记录、填补等方法进行处理。数据填补采用KNN进行,合并用药中,使用人数<10%的药物将会被剔除。分类变量采用One-hot编码,连续变量采用Z-score做归一化处理。 ③同时对于重复的记录进行去重删除操作,减少训练模型所用的时间。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

The criteria for judging whether the data are available or unavailable are recorded in the study plan (such as lack of time for blood sample collection, dosing data without relevant concentration information, and concentration information without dosing data). Establish standard operating procedures to standardize the quality and effectiveness of management data (including data input, storage, verification, correction and retrieval) and statistical processing methods and software. 2.
